"Died, no test results" should be 13

That is, if I did my addition correctly.

Gardner, the differences in totals can be because
I began a week or two ago, also adding the names of the patients that were listed as having been "tested".  This does not mean they have symptoms or are suspect, but I wanted their names added to the spreadsheet so we could see who was being tested, and then turning up with symptoms.  Which paid off, because the HCW's who we had listed as tested were then admitted with symptoms.

The summary table is set up so that when you put letters and numbers in certain places, they automatically tally up the table.

I have to remove the "count" number, but also remove the "letter" it would correspond to. When we list a group of patients the same thing happens.  I found one place where the "7" was still listed in the count column for a row which read "7 patients tested".  I will go back tomorrow and double check all those areas and resend the summary table to AnnieB.  I will also have to check the rows and make sure a formula was entered for April.  That may also have something to do with the count.

To the far right, are new additions, which include the dates when we post the article in a diary, and date for tests results (color coded).  Also I am adding the links to (some ) of the original articles so if anyone want to go back and re read the indo article.  The links for that patient are pretty much in one location. (not all links are copied to the spreadsheet).

Metrotvnews.com, Balikpapan: six people from one family from the Compass Regency, the Grogot Land, East Kalimantan, was stated by the bird flu negative. Moreover, one family that could be treated in the Kanuyoso Jatiwibowo regional Public Hospital, Balikpapan, East Kalimantan, this was permitted to come home.

One family, Haris and the wife, Wahidah togethered with their four children, Erwin, Erlin, Edwin and Erpin was reconciled to RSUD Kanuyoso by the doctor from RSUD the Commander was as Old, the Grogot Land after being expected terjangkit the bird flu virus.
They were at once put into Keramunting RSUD Kanuyoso isolation space and underwent the maintenance, including being taken the sample of his blood. However, was based on results of the inspection rapid test, on Wednesday (4/4), one family was stated by the bird flu negative, so as to be permitted to come home. According to the patient was expected by bird flu, Haris, his family was reconciled to RSUD Kanuyoso, after his two children died in time almost simultaneously. He then regretted the decision of the so fast doctor to state his family was expected by bird flu. (DEN)

I am having a hard time believing this one.
The whole family has symptoms, their chickens die, 2 of their children die in a period of 2 days apart and because the "rapid test" shows negative, they go home. The doctors in Indonesia know the rapid test is only 30% accurate. If the family members are in fact positive then they are still carrying the virus, which means they could infect others. To my way of thinking this is just totally crazy. I think the Indonesian government is orchestrating as big a cover up now with the cases of bird flu as China is doing. It just makes me so angry and frustrated.

I did not know that. No wonder we've been getting so many "neatives".
I agree Carol@SC, this is totally crazy. But I'm not so sure is it is a cover up. I think this is the only capacity the health care has. they simply can't keep everyone who is "suspected BF". They simply don't have the time, or resources. Imagine is 20 people a day were showing up with "suspected bf" symptoms. They would be overwhelmed in a short peorid of time. And because there is no epidemic or pandemic, only seasonal flu, they must assess each one as quickly as possible. Get them in and out asap. Each one is tested, test is negative, and that's enough for the doc. "You can go home."
We must keep in mind how poor their resources are. Having said that, I do think all this time they have been letting  some false negatives go home. With the test having only 30% accuracy rate, it is enevatable.

Possibly the family insisted on going home-
to make burial arrangements for the two dead children.  No, I don't think they should be released if theres a good chance of them still being infected but if the tests were negative (even though not accurate) I can see where the hospital would be under pressure to let these poor souls go home.

It also sounds like the father was not at all happy
at being given the suspect bird flu diagnosis, and then having it changed to negative. 

He then regretted the decision of the so fast doctor to state his family was expected by bird fluj.

No doubt one's popularity in one's community does not go up when one is diagnosed as H5N1 positive.  And, even more serious, remember two of the children died, so that would be very scary for the neighbors (anywhere in the world).

Indo Dr's Association Chairman on Rapid Tests

Indonesian Doctors Association chairman Fahmi Idris said. Fahmi said diagnosing avian influenza was not easy for general practitioners because its symptoms were similar to other viral diseases. "We would like to have rapid tests, but so far, in other countries, the tests have proven to be ineffective.

what makes me think, in East Kalimanten, far far away from Jakarta, East Java, and such,  that they would have rapid tests available?

crazy like a fox, maybe...
Chances are the Tamilfu is working, and that's what they are counting on (and keep counting on).  I think they are betting the farm on the Tamiflu efficacy, though.  Those first two children in the family did not get Tamiflu early enough (and may have infected each other as indicated by the onset dates, although that's still at issue).  The rest of the family would have been given Tamiflu immediately.  That's key to the whole plan. 

From all we've seen, it looks like people are not infectious if they've been given Tamiflu in time.  It seems to tamp down the virus not just for the sake of the tests, but it does seem to prevent the person from passing on the virus to others as well.  These are just my annecdotal observations, of course. 

So long as Tamiflu keeps working, and we have enough of it, they'll likely keep going along just like this. Why not?  In their minds, it's "worked" so far.

From all we've seen, it looks like people are not infectious if they've been given Tamiflu in time.  It seems to tamp down the virus not just for the sake of the tests, but it does seem to prevent the person from passing on the virus to others as well.  These are just my annecdotal observations, of course.

"Indonesia Health Ministry numbers are 93 cases and 72 deaths, according to the local press, as of April 5, 2007.

"The current WHO numbers are 81 cases and 63 deaths, not updated since Jan 29, 2007. That may be updated moving forward with new agreements with Indonesia on viral genome sharing, but it is unclear as far as the WHO numbers what will happen with the Jan-March cases."

That's my summary based on your collective work, and I am asking you all check it for accuracy.

Is Indo erring on the side of caution or noise or whatever?

How are you differentiating?
How are you differentiating those who had symptoms for a long time (and how is "long time" defined) from others?

IMO, the benchmark should be date of symptom onset if that info is available. Certainly someone can be symptomatic for a long time before showing up at the hosptial, so that could be a variable.

but the issue with WHO is they haven't gotten samples since Jan 29 to verify, based on the dispute.

comparing egypt with indonesia..
Both countries are having an upsurge in cases, IMO. But Egypt's have been very young children and mild symptoms, kids survive. Indonesia have been the usual suspects (young adults to middle age) and the same or even increasing virulence. It almost seems like we have two very distinct strains developing in these two areas,and both seem to be becoming more infective.

Could our scientists weigh in on these observations and tell us what they think these trends might portend?IMWTK.

I was able to use the admission and hospital transfer dates to show that the "not identified" case and Rizky were the same case.

the specifics are beyond me ;-)
I'm afraid, but suffice it to say that in the WHO classification, the Indonesian virus belongs to Clade 2 subclade 1, while the Egyptian virus belongs to Clade 2 subclade 2.  Go to this link http://www.who.int/c... open up the file, there's a good summary phylogenetic tree on page 4 which shows the antigenic distance between these 2 subclades.

thanks, susan. I looked, and just got confused.
I don't really know what constitutes a "clade": obviously changes in the nucleotide sequences in key antigenic sites. But it would seem that clade2 subclade 2 in Egypt has now evolved into at least two distinct subclades, the one infecting children with (seemingly)less virulence and the one infecting adults that is somewhat tamiflu resistant and seems to be becoming more virulent. This WHO phylogenetic tree doesn't reflect these changes, nor show what they are. I also wonder to what degree changes in the antigenic sites actually reflect changes in the virulence and infectivity. Do they necessarily go hand in hand. I guess I need to take a trip to Niman land, he's probably got this data.

the most important issue
for practical purposes is whether there is enough cross immunity for protection with vaccine made from a different subclade.  Otherwise, these classifications are sometimes open to dispute depending on which points were used for 'rooting'.  More recently Derek Smith and others have started to use other techniques to produce antigenic maps, which would probably be even more useful, as discussed in the abstract of this paper Mapping the Antigenic and Genetic Evolution of Influenza Virus

The antigenic evolution of influenza A (H3N2) virus was quantified and visualized from its introduction into humans in 1968 to 2003. Although there was remarkable correspondence between antigenic and genetic evolution, significant differences were observed: Antigenic evolution was more punctuated than genetic evolution, and genetic change sometimes had a disproportionately large antigenic effect. The method readily allows monitoring of antigenic differences among vaccine and circulating strains and thus estimation of the effects of vaccination. Further, this approach offers a route to predicting the relative success of emerging strains, which could be achieved by quantifying the combined effects of population level immune escape and viral fitness on strain evolution.

Here is an example of what an antigenic map would look like.  Just to illustrate that there are more than 1 way of figuring things out, and that this is all evolving science.  Just so you can be aware of this.

For example, we don't really know that the viruses that infect children and adults in Egypt are really antigenically different.  The same virus can have different disease patterns at different age groups.

WHO will only include cases confirmed at a
WHO accreditted laboratory.  Indonesia stopped sending samples so WHO doesn't count them.

The count we/I have maintained since late January still requires that the Indonesia Dept of Health confirms the case is positive.  We did have one where the local hospital said positive but the Indonesia DoH did not confirm - it is noted as such in the Case Information Diary.

Of greatest concern to me is that virtually all other countries base their pandemic plan and subsequent actions on the WHO information / data / status.  As the Indo info is not being reflected in the WHO numbers, this will not give a full "picture" to other countries.

Who Vows Vaccine Stockpile for Poor States

Responding to calls from developing countries for the fairer distribution of bird flu vaccines, the World Health Organization will create a global stockpile of vaccines for poor countries.

Director general of the WHO, Margareth Chan, said Wednesday the global vaccine reserve would be of great importance for developing countries in their fight against avian influenza.

"We will look at how to develop global stockpiles of vaccines, particularly for developing countries," Chan said after a meeting here with President Susilo Bambang Yudhoyono.

Meanwhile health officials confirmed a housemaid died Sunday of bird flu, marking the country's 72nd case of avian-influenza related deaths.

Chan said the WHO would gather resources from rich countries, donors as well as large drugmakers to make the plan work.

An assumption patient of bird flu that was treated in the doctor's Hospital Mawardi, Solo, of Central Java, recently had finally died.
Now in Jakarta, Rizky was still being treated in the central Hospital of the Infection Professor Sulianti Saroso, Sunter, Jakarta North.
Rizky was the resident of the Pegangsaan region, Jakarta the Centre.
Doctor Sardikin, Director RSPI Sulianti Saroso, stated results of the laboratory inspection pointed out this patient positive was infected by bird flu.
The patient was 25 years old this was still feverish high and the cough as well as contracted the inflammation of the heavy lungs.
Was based on the family's information, Rizky indeed maintained birds in his house.
However, as far as this is concerned, was not yet known was not him the poultry that died suddenly.
As for since spreading in Indonesia mid 2005, till at this time was recorded by 93 cases of bird flu with 73 including dying [read: WHO facilitated the Cheap Vaccine Research]

  Metrotvnews.com, Bandung: the Handsome Sadikin Hospital (RSHS), the Bandung City, West Java, again treated a patient suspect bird flu.
The patient had the initials P, villagers Mandalasari, Cikancung, of the Bandung Regency, was reconciled with the clinical sign similar to the bird flu sufferer.
He suffered the high and breathless fever after the previous day three chickens that pelihara him was found died.
The patient could be treated in the Majalaya Community Health Centre, the Bandung Regency.
He was afterwards reconciled to RSHS because the fever that was suffered by him will never improve and have the story of contact with the poultry died suddenly.
Currently, the patient was treated intensively in Poinciana Tree Space, RSHS. The Total patient suspect bird flu that currently is treated in space of the Poinciana Tree isolation improved to two people.
The patient beforehand had the initials the F name, the resident Street Ciparay, the Stone, Bandung, that more previously had been treated since the end of March set.

Indonesia, Egypt to develop bird flu vaccine
JAKARTA, April 5 (Reuters) - Egyptian vaccine maker Vacsera is looking to team up with an Indonesian company to develop a bird flu vaccine for humans, an Egyptian minister and a company official said late on Wednesday.

"With regard to avian flu, the Egyptian company involved in cooperation for producing or manufacturing a vaccine with Indonesia is Vacsera," Egypt's minister of international cooperation, Fayza Aboulnaga, told a news conference.

Vacsera's chief, Salwa Seddik, said the firm had approached Indonesian vaccine maker PT Bio Farma for joint research and development of a vaccine, but did not give details.

"We have contacted the Indonesian health ministry and Bio Farma. It is at an early stage but we do hope we can start as soon as possible," Seddik told reporters.

"We don't know until now where the vaccine is going to be produced but we have the facility in Egypt," she said.

Indonesian Trade Minister Mari Pangestu said the government has yet to decide which drug company would be involved.

There is no commercially available bird flu vaccine for people but several major drug firms are working on one.

Rizki Passed Away
Metrotvnews.com, Jakarta: Another the positive patient bird flu died.
He was Rizki Julianti, 14 years.
Rizki died in the Infection Hospital (RSPI) Sulianti Saroso, Sunter, Jakarta North, on Thursday (5/4).
Beforehand, Rizki was treated in the Saint Carolus Hospital, Salemba, Jakarta the Centre.
However, his condition got bad, so as to be reconciled to RSPI Sulianti Saroso on Monday set.
Fortune was stated positive bird flu after beforehand underwent the inspection of the blood test totalling twice.
When arriving in the hospital, fortune has been affected by pneumonia.
From relatives's information of fortune, their house in Street Sibayak, Pegangsaan, Central Jakarta, indeed maintained the poultry.
However, not there is any the poultry that was sick and died.

Recon:  Saint Carolus Hospital

The other "R" was reconciled from a Catholic Hosp.  If it wasn't for the age difference.....they were both admitted on the same day....I don't have a village for "R" (15).  It could be a typo with the age.  But what about the two different hospitals?  I'm going to go back and look at the article that says "R" is in Friendship.

Wow - where is the difference?
The first report of the "not identified", which became R, was an Islam hospital which became a catholic hospital which became Saint Carolus.  Islam and catholic could be translator problems; Saint Carolus is a 'religious' name.

On transfer, the first information was to Persahabatan hospital - I had not heard of this one before.  The regulars assured me this was Friendship.  It turns out that the transfer was to Sulianti Saroso.

I do but usually I will give someone a hat-tip
and bring a new case over.  Or I'll make a notation of the site I got it from.  So I don't understand why I wouldn't do that, especilly coming from Bali.  I go to only 4 sites:  Wiki, PFI,  CE  and FT.  I tried to do a search yesterday at CE and FT on this Bali case, but I never ever have luck when doing searches for info at these forums.  So, maybe you know how to do it better than I.  I had to have got this from one of the three.  I scrolled through PFI yesterday, but couldn't filnd it.  But I was looking for "Bali" in the subject line.  Not looking at each post.  I vaguely remember it possibly being read a good week or so after the admission date.

the Balinese are Buddhists, and therefore traditionally segregated from the main Islamic population in marriage.  That was another reason why I wondered whether the genetic predisposition issue may be playing a role in the outbreaks.  But it's not something that is easy to determine, I don't think.

The information given on them is different. Aisyah is listed as 11 months old and Alfina is listed as 5 years old. They were admitted the same day (4/3) to RSUP M. Djamil Padang.  I have the 5 year old as being reconciled from RS Solok. I will send a copy of my spreadsheet to you tonight.

Metrotvnews.com, Makassar: A male child have the initials "Mt" that was one years old it was suspected was infected by bird flu.
Since last April 2, Mt underwent the intensive maintenance in the Hospital (RS) Doctor Wahidin Sudiro Husodo, Makassar.
Initially, Mt experienced the fever with the temperature of the body 38 levels celsius, the cough and breathless.
To confirm Mt terjangkit bird flu or not, the medical team of the Hospital Doctor Wahidin Sudiro Husodo took the sample of mucus and blood.
The sample of this blood was sent to Jakarta.
MT had the story of contact with the poultry.
Last December, dozens of chickens around his house in the Bangkala District, the Manggala Subdistrict, Makassar, was found died suddenly.

The age was listed in one place as 2, but in another as 4.  The hospital was only listed as the public hospital, and the home location was Tanjung Pinang in Riau.  There was never any more information, that I noticed, anyway.  Maybe she was transferred to Jamil?  That seems so long ago, but it's really just about two weeks.

JAKARTA, Indonesia (AP) -- An Indonesian teenager died of bird flu in the bustling capital, bringing the toll in the country hardest hit by the virus to 73, a health ministry official and doctor said Friday.

The 15-year-old girl was admitted to the Sulianti Saroso Hospital for Infectious Diseases on Monday and died late Thursday, said Sardikin Giriputro, a doctor at the facility.

"By the time she arrived, it was too late," he said. "She died after experiencing multi-organ failure."

Health Ministry official Muhammad Nadirin confirmed that the girl's lab tests showed she had contracted the H5N1 virus, apparently after coming into contact with infected pet birds at her home in central Jakarta.

Kelly, the ages can vary by one year because
of the way ages are expressed in this part of the world.

Some use the "western" system where you count each year of life after it is completed.  Others use the system where your age is the current year you are in - i.e. you are born as "one".

So, you would say you are in your fifteenth year (will turn fifteen next birthday) and others will say you are fifteen already ;-)

This just adds to the confusion I know but ages within 1 year are basically the same.

It seems as if we are seeing one new death almost every day.  It used to be hard to catch H5N1.  Now, the news stories are filled with cases.  The new normal would have made everyone queasy only a year ago.

This "new normal" has snuck up on us, hasn't it?
It's like the frog in the beaker...if you heat up the water real slow, the frog will just sit in there until it boils to death, whereas if you drop it in a beaker of hot water it will jump right out.

We are seeing a slow but steady increase in cases, but it's not rapid enough to make us panic. It is sneaking up on us incrementally, this pandemic. As Kelly says, today's figures would have made everyone queasy a year ago: now we just watch nervously, discuss meanings, and wonder about cases as it spreads and grows. My question: when do we jump?

Always have a plan B.

The problem is that the H5N1 patient reported no contact with sick fowl
The last patient we know of who was at RS Islam Hospital, Central Jakarta, was Sukaesih (23 F ).  She had symptom onset on 3/28 (fever, cough, diarhea), was transferred to RS Persebahatan Hospital (Friendship Hosptial) on 4/01, and died the same day. 

"The official said that it was not clear whether the woman had historical contacts with fowls."

They are still using whether or not a suspect patient has had contact with sick birds as part of the diagnostic criteria to determine whether someone is to be considered a suspect-H5N1 patient.  The official WHO diagnostic criteria still include this question and an affirmative answer as properly necessary to such a diagnosis. 

So, if someone says that they had no contact with sick birds, that no birds have died near them nor in their neighborhood, they are retained and are treated (and are likely housed) as ordinary patients until some kind of sharp deterioration makes the medical staff suspect H5N1. At that point, they are usually tested and are quickly transferred up to an H5N1 reference hospital.  Before that time, though, if a patient reports no sick bird contact, they are just a regular patient and no special precautions are taken. 

Because we are seeing an increasing number of cases where there is no known contact with sick poultry reported, they should really consider changing the diagnostic criteria.  The H5N1 vectors at this point could include birds who are asymptomatic, cats, mice, and other humans.  After all, the definition of a "pandemic" is a disease that is caught from other people, not from birds.  ;-)

WHO Case Definitions
Cases without contact with sick or dead birds can be called "suspected," per WHO case definitions.  ersonally, I think it's time that Suspected H5N1 Cases, as defined by WHO, stop requiring everything following the "AND" below. It should be enough in Indonesia to SUSPECT ANYONE "w/ unexplained acute lower respiratory illness w/fever >38 degrees C, and cough, SOB or difficulty breathing." 

If you limit suspected cases to those who get closer than 1 meter to a person/bird/other mammal who is positive, you never find the start of the pandemic...Because your definitions exclude you from looking for it!

Sure, but they're not considered suspect cases
unless they report having been near sick poultry, or poultry that has died.

No sick birds nearby = no suspect H5N1 case, regardless of the symptoms (until the symptoms become unmistakably severe, and then it's usually too late).

I posted a few weeks ago...
When an indonesian hospital reported that they were doubling up pts. in "isolation."  I got a reply saying that by the time a patient gets to the hospital, they're usually past the contagious time, since pt. is contagious for 1-4 days before symptoms appear, then for 1-4 days after. 

I didn't like the reply, because it seemed it assumed too much.  And infections days, pre- and post-symptoms could range from 2 to 8.  Too much variability for me not to be concerned about this "doubling up."

If patients are not contagious, why would anyone put them in isolation to begin with?

I would not be surprised
to hear that the neighbor, Ruph'ia (43) has been admitted.  A temp of 39C is usually ground for hospital admission, especially when H5N1 is susptected. 

I also wonder what kind of provisions they make for isolating these patients who are under observation if they are remaining at home.

Ruph'ia's fever was already really high
so if Tamiflu is administered to Suwandi and Ruph'ia, they may still both test positive. 

What is also notable in this suspect cluster is that Suwandi (8) has been admitted to RSUD Dr. Soetomo, Surabaya - the same hospital our cluster of HCW's (the two doctors, the nurse, and the midwife who had treated Wetono and who had suffered his same symptoms), disappeared into. 

There's a lot of activity around Surabaya lately.  I hope we hear further word on this cluster of six surrounding Suwandi.

Suwandi is in ICU, and his "trombosit and his leucocytes have descended."  This, again, is another patient who has had a rapid deterioriation. He had only fever on Tuesday, began with bronchial symptoms the next day, and by 13:00 on Thursday had to be taken to RSUD Dr. Soetomo where he is in ICU after experiencing a descent in "trombosit and his leucocytes."  Again, Wetono, who was treated in the same hospital, also deteriorated rapidly, and had experienced the "trombosit and leucocyte" that "descended drastic" before his death.

They haven't found any sick poultry yet
around Suwandi's neighborhood, as far as I know.  They sure are looking for it, and they are disinfecting the nearby poultry pens anyway.  The closest pen is reported as being 50 metres away from Suwandi's home - "the distance of the patient's house from the livestock breeding pen in his village around 50 metres." Suwandi is reported as having contact with poultry, but no poultry has been reported as being ill or as having died so far.

The news report that bwg in MT posted above says that Gresik Regency reported the death of birds from H5N1 in early March:

"...at the beginning of last March, the Gresik agricultural Service proposed the extraordinary condition (KLB) bird flu, followed the death 17.000 poultries in two villages, namely the Dermo Village and Sirnoboyo. Both of them entered the Benjeng Subdistrict."

If they had found sick birds near Suwandi's residence, they no doubt would have stated that.

West Java Regulates Poultry to Curb Bird Flu

West Java Governor Danny Setiawan has signed into law a regulation aimed at curbing the spread of bird flu in the province, three months after neighboring Jakarta passed a more stringent regulation.

Unlike Jakarta, the West Java regulation does not ban backyard farming or impose sanctions for violators. West Java has been hit the hardest by bird flu, with 27 deaths out of 29 reported cases of the H5N1 virus. Nationwide, 73 people have died out of 93 confirmed cases.

Rahmat Setiadi, the head of the West Java Animal Husbandry Office, said in Bandung on Thursday the governor signed the regulation this week in response to an earlier presidential instruction to intensify the fight against bird flu.

"West Java is still reporting bird flu case due to the lack of monitoring of poultry. We hope to solve this problem with this gubernatorial regulation," he said.

The regulation states that all poultry must be kept at least 2.5 meters away from all houses, and should be placed in an area that receives direct sunlight.

It also says the technical details governing the movement of poultry in the province would be agreed upon by West Java's 25 regency and municipality administrations.

However, the regulation could prove to be toothless as it does not contain sanctions for violators.

"It is mentioned in the regulation that a bird that tests positive (for the bird flu virus) should be destroyed. But when it comes to healthy pens, all we can do is offer advice to the public," Rahmat said.

He acknowledged the regulation might not be sufficient to eliminate the virus from West Java, but expressed optimism it would help reduce the number of bird flu cases in poultry.

Rahmat said that in the first three months of this year, more than 19,500 chickens had died in 24 regencies and mayoralties. Bird deaths were recorded in 189 villages and 132 districts, while more than 12,000 birds being destroyed.

Meanwhile, a 29-year-old man suspected of having bird flu died at Dr. Moewardi Hospital in Surakarta, Central Java, on Thursday.

Test results are expected Saturday to confirm whether he had the virus. In the meantime, his remains are being handled as if he died of bird flu.

The man's coffin was not allowed to be opened by relatives before his funeral in Sapen village, Sukoharjo, also in Central Java.

"We asked all those who bathed the body to wear masks. We also placed the remains in a coffin and sealed it so the family would not open it. These are just safety precautions," a doctor at the hospital, Suradi, told journalists.

A cousin of the victim, Sumardiyanto, criticized the hospital's handling of his relative. He said his cousin arrived at the hospital on March 30 and was placed in a SHARED ROOM, being moved to isolation only four days later.

He claimed staff at the hospital panicked when they first suspected his cousin had the H5N1 virus. "All of the patients (in his shared room) were moved out and the room was sterilized. After that (my cousin) was taken to an isolation room."

Dr. Suradi said the victim was initially thought to have dengue fever. "But it's not true that we did not give him proper attention. We used standard procedures in dealing with avian influenza suspects."

The victim purchased about 200 kilograms of chicken meat at the Semanggi traditional market on March 15, which is when he is suspected of having contracted the virus. A day later dozens of his neighbors' chickens died.

The monitoring is of 6 houses
which we could assume are those nearest to Suwandi and Rupi'ah - the neighbouring houses of these two.

Because there are further signs of illness in these two houses, the Health Dept has asked the monitoring to be extended to more of the village.

I suggest it is a bit like the culling process - there is a radius given within which all poultry is culled.  In the case of humans, it appears they are establishing a radius within which all houses are monitored?  This could well be because the infected poultry are free ranging?

The key word is that they are actively monitoring - I think this is a huge advance - previously cases had to be brought to the attention of the medical people.

If we had a capable native speaker translating, we could at least know that most of the villagers feel they are sick. It's not possible to  tell for sure with a machine translation. Also, are they really sick, or just nervous?

I do know that this did not improve my estimation of the situation in Indonesia. Of course, it wasn't that good to begin with.

Your real experts would be the three newshounds that have worked this beat far longer than I have: Commonground, Influentia, and Pugmom (in alphabetical order, LOL)

This sentence, from one of Commonground's reports, worries me also: "In the meantime, followed the RO death, the head of the community health centre 2 Mojolaban, Dr Sriyono MKes, stated his side checked 11 people, including the family's RO member, that at this time was suffering was sick flu." What I got out of reading this article made me feel really uneasy. I guess it is because we haven't seen much monitoring going on in these villages and now it seems they are giving this one perticular area intense attention. The attitude in Indonesia was so lax for so long that any changes in their actions makes ones ears perk up. Also, it seems to me that we are seeing more clusters now than before. The HCW's getting ill lately is a big change in the situation and very unnerving.

I thought about commenting on this one last night, but since I'm normally far more jumpy than most on here, I did not want to alarm myself any further.  I was hoping that another calmer mind than me would be able to interpret the translation in a less alarming way than how I was reading it.

This part is so ambiguous, it really could be read and understood either way.  Either the virus is with RO, the patient only, and the others are just worried, or the 11 people checked are also 'at this time suffering was sick flu'

We had some reports (?) regarding burning the chickens and how dangerous it was to be near the smoke.  But we have never had any reports, or insinuations of a whole village being infected.

I agree that what you posted can be alarming.  I'm not sure right now, which way to go, so I'm  hoping to get more articles.

Things have changed, since we had the report of the HCW's.  They are watching villages now....and with poultry being infected with H5N1 all over Indo at an increasing rate, this was to be expected (more people getting infected by touching, culling infected birds).  We have to get more information.

and make a wild but educated guess that the Indonesians have confirmed H2H and are now on the lookout for whole villages, not just blood relatives of a sick person.

I agree with you about the HCWs and how that one group of caregivers-turned-patients became the pivotal turning point for H2H.

I think tptb in Indonesia are increasingly aware of the h2h factor but are trying to keep it hidden as long as possible. I also think WHO knows more about this than they are telling the public. If that is right then they should be ashamed of themselves for keeping so mum about it. The situation in Indonesia with the poultry, humans and Lord knows what other creatures is deteriorating. At this point, I think "the horse is so far out of the barn that it's going to be impossible to reign him back in".

I think the anxiety and frustration is for others that don't know and aren't prepared.
If WHO and tptb would stop playing politics and tell the world what we here already know for 99.9% certain: that this pandemic is imminent, that H5N1 - not "some other virus, sometime in the future" - is going to turn into a globally catastrophic pandemic within the next couple of years, and very likely within this year; and that every individual needs to begin preparing now for that eventuality, maybe we could stop feeling so anxious and helpless.

Most of us here have friends, relatives and co-workers that we care about that just don't take our warnings seriously because tptb haven't made any such pronouncements.  So we look at them and see dead people.

I feel better now.  Today was especially bright and warm, and so, to take out my frustrations, I went outside and dug up the hard ground to put more plants that I had bought at the nursery.  I got one brocoli plant, two carrots, and six swiss chards to join my crookneck squash and my eightball squash.  There were a couple of pods of garlic that had grown some green shoots, so I stuck those in the ground.  I also had some potatoes that were starting to grow, one big baking one and two small red potatoes, so I stuck those in the ground too.  I hope they do something.

Nothing like gardening to take away the stress and relax the mind.  :)

I suppose they could all have pnemonia.

And now they want to monitor ALL THE HOUSES IN THE VILLAGE.  Well then, they must think it's more than pnemonia, or else they wouldn't be ratcheting it up a notch to go to all the houses in the village.  Well.

If you read through the recommended actions for Phase 4, they're exactly what's happening in Indonesia right now.  Starts on page 29. 

It's the first time the plan says to use antivirals for early treatment and to consider antiviral prophylaxis for close contacts.

Seems like Indo's laying out the Tamiflu blanket, and that's not a Phase 3 procedure.

Also, phase 4's overarching goal is about buying time, INCLUDING vacine development.  What was it again that Indo and the WHO have been talking about since January?  Was that something to do with vaccine availability?  I seem to vaguely remember something about that....

I don't know about anyone else, but I'M IN PHASE 4.

that says burning chickens cause cough, breathless, AND FEVER.  I don't think this has anything to do with burning chickens and the villagers are scared and are grasping at straws.

It sounds to me like someone is trying to do damage control. They don't have the results of the PCR back yet. The test they are talking about sounds like the rapid test.I imagine people over there may be even more worried than we are and they are trying to reassure. I wonder what the lab values really are. The news had already leaked out several days ago about the thrombocytes and leukocytes, so they have to work with that information. I don't know that they are lying,but  I don't trust them to give the truth to the public without a great deal of spin.

Could someone who understands this sequence well please summarize?
I think that this particular situation in Gresik is/was serious enough that a causal chain summary linking the suspect cases would be very useful for our analysis.

I myself tried to backtrack through the postings, and only got more confused.

Was Suwandi originally suspect because of exposure to another case that was positive for bird flu? Wasn't that why they burned all those chickens? Or is that another situation?

  Were these other cases (Rupiah, Kasmi, Pendi et al) exposed by Suwandi or by the other case?  If we have all this kind of thing tracked out chronologically and otherwise, we can make a better judgement as to whether the fact that Suwandi tests negative is really significant (and believable) or not.
