JAKARTA, Aug 22 (Reuters) - An Indonesian woman from the popular tourist resort of Bali has died of bird flu, a health ministry official said on Wednesday, the second confirmed death from the H5N1 virus on the island.

Joko Suyono of the ministry's bird flu centre in Jakarta said the woman, who died on Tuesday in a hospital in Bali's capital Denpasar, tested positive for H5N1 after a second test.

The woman, a 28-year-old poultry trader, had suffered from high fever and breathing problems before being hospitalised near her village of Tanah Lot on Saturday. Tanah Lot on the south-western part of Bali is popular with tourists for its postcard sunset view and a temple that sits on an outcropping of rock in the ocean.

Sorry if this has been reported on already, but has the Jembrana District (sometimes also referred to as regency or province) been taken off of the 'isolation' status. 

Jembra is evidently a district of Bali Province and was placed under some sort of isolation by the Governor back on August 14. http://www.newfluwik...

S'pore sets up bird flu control system in Indonesia's Tangerang city

SINGAPORE : Singapore has established a bird flu control system for its bird flu pilot project in Indonesia's Tangerang city.

Charun Sopandi, head of the husbandry division at Tangerang's municipal agriculture and husbandry office, said two volunteers would be stationed at each of the more than 100 villages.

They would get first-hand information on suspected bird flu sufferers, and would then inform the city administration and the central government of suspected bird flu cases for quick treatment.

The Indonesian official said Singapore has also provided Tangerang with 208 motorcycles, 13 disinfectant sprayers and two computers equipped with special programmes to record bird flu cases. http://www.channelne...

If pandemic breaks out, Singapore is the next stop, then the rest of the world. That hospital may be partially funded by Singapore.  Japan is also doing a deal with Indonesia to get vaccines.  Unfortunately news in Singapore may be censored, so we may not hear it quick enough.

Denpasar, Bali (ANTARA News) - The conditions of I Nyoman Mar (58 ), a suspected bird flu patient being treated at Sanglah Hospital here, are improving, hopital sources said.

Nar, a resident of Banjar Pasar village in Jembrana district, is no longer using a respirator and the patient`s body temperature has also dropped, a hospital official told ANTARA here on Wednesday.

Admitted to the hosptal three days ago, Mar was beginning to enjoy food again and able to sit up.

But the team of doctors who have been treating Mar continue to monitor developments in Mar`s state of health closely.

The hospital has set up a special ward to treat bird flu patients. So far, six suspected bird flu patients have been treated at the Nusa Indah ward with two of them, namely Ni Luh Sri W (29) from Jembrana and Ni Sri N from Tabanan, having died recently.

The Bali provincial administration has since 2003 taken a number of precautionary measures to prevent possible bird flu outbreaks from spreading on the island. It has set up a special team of doctors to deal with the disease, special wards and equipment at Sanglah hospital to treat suspected birdflu patients.

The administration meanwhile would inject Rp1.8 billion more in funds to increase the capacity of Sanglah Hospital`s Nusa Indah ward in anticipation of a possible increase in the number of birdflu patients brought to the hospital.

It has also named two other hospitals -- one in Tabanan and another in Gianyar -- to also become reference hospitals for bird flu sufferers.

JAKARTA (Reuters) - The Indonesian military will help in the fight against bird flu as part of the country's efforts to stamp out the disease that has killed 63 people, the bird flu national committee said on Friday.  The military will set up bird flu treatment centres at two army hospitals to ensure wider access to medical care people infected with the virus, the committee said in a statement. In addition, the military will also support national and regional bird flu control efforts in remote areas in the sprawling archipelago, which has the world's highest human death toll from the virus. 

"The military will also work closely in developing Indonesia's contingency plan for a possible pandemic by holding regular simulation and field exercises," Bayu Krisnamurthi, the head of the committee, said.  "We will coordinate all military services related to bird flu control programmes."

Separately, Aburizal Bakrie, the coordinating welfare minister, said the government will set up 150 mobile units which can be sent to areas with bird flu outbreaks. 

"These mobile units can move faster to do, for example, spraying disinfectant in outbreak areas," Bakrie said. "There have been no reports of bird flu human case in Jakarta for a month since the ban on keeping poultry in housing."

Didn't they open up some military hospitals and employ the military to cull poultry earlier this year? We never heard much about it afterwards, if I remember this correctly.

There was some mention of the TNI several months ago.

But then the story disappeared.  I kept looking for any news of the army and how the government intended to use it, but I couldn't find anything until today.  Just got lucky.  I've assumed that they are using the army whenever they quarantine an area, to stop border smuggling, for poultry culling, and possibly some military hospital utilization. Until recently the Indonesian army and the police were under the same jurisdiction, and I'm sure the old connections/cooperative ventures are still there.

Without involving rural communities in the fight against bird flu, Indonesia will not be able to control the highly contagious disease, which has ravaged millions of poultry and already killed more humans in the archipelago than anywhere else in the world, the United Nations Food and Agriculture Organization (FAO) warned today.

In an effort to control the H5N1 virus, which experts fear could spark a lethal global human pandemic, FAO has helped to train teams of local veterinarians and para-veterinarians in participatory disease surveillance and response (PDS/R) techniques in a country that numbers 240 million people spread over 6,000 islands.

"Almost four years after the first outbreaks of highly pathogenic avian influenza in Indonesia and more than 80 human deaths, there is still an urgent need for community members to be involved in the detection and reporting of sick and dead poultry," the agency said in an update.

Veterinarians and paravets are engaging community members, tapping into their local knowledge and involving them in control efforts. Villagers are also trained to detect and report bird flu cases in poultry and be responsible for their own safety and that of their families.

FAO is planning to extend the training of more PDS/R teams at local government level. "We will be expanding PDS/R further to cover Sumatra, Kalimantan, Sulawesi and Papua," James McGrane, FAO Indonesia's Avian Influenza Team Leader, said.

Surveillance and response teams are currently operating in 168 out of the 444 districts. So far 1,200 officers have been trained. PDS/R capacity has been established in all districts of Java, and in the provinces of Bali, North Sumatra and Lampung, home to almost 70 per cent of the population. Provincial PDS/R capacity has also been established in all provinces of Kalimantan and Sulawesi.

"Villagers are like detectives in the field. They are directing us to areas where there are H5N1 outbreaks among poultry," Ibu Azmiyati, a veterinarian with Tangerang district Livestock Services Department, said. "Without the help of the communities, we would be lost. There are simply too many backyard farmers and village households."

She was identified positive of avian influenza (AI) based on observation of Balitbangkes and Eijkman Laboratory. Therefore, there are 2 AI positive cases dead in Bali. Cumulatively, AI case in Indonesia reaches 105, with 84 people dead. The Case Fatality Rate (CFR) is 80%.

She had her onset in August 2007 and was hospitalized at Sanglah Hospital, Denpasar on August 8, 2007 with difficulty in breathing. She had contact with chicken previously, while rapid test to chicken is positive of H5N1 virus.

This latest information is received by center for Public Communication from Dr. H. Nadhirin at AI outbreak post, DG of Disease Control & Environmental Health MOH on August 22, 2007.

There has not been effective drug for AI. The recent one is oseltamivir, which only functions to obstruct virus growth. The only effective way is by preventing AI virus infection.

AI transmission still comes from avian. Since the huge risk factor of transmission among human, people are suggested not to keep avian for awhile or separate avian from settlement.

When finding sudden dead or sick chicken/avian, people should immediately report it to RT/RW, head of village, health center or local health/farming offices.

To prevent and protect from AI infection, people should:
o not touch dead or sick avian, or else, immediately clean body with soap.
o wash hand and kitchen utensils with water and soap, and cook avian meat and eggs well.
o separate avian from human as well as new avian from the old ones (at least 3 weeks)
o go to health center or hospital when finding flu and fever symptoms, especially after having contact with avian.

It is a jumble.

There were several words that I simply couldn't translate.  I used every conceivable translation website that we have at our disposal, spending about two hours on this story, and I finally concluded that these words might be a regional dialect or slang.  Or perhaps they were misreported in the original story.  My take on this story is that there are some major disagreements within the medical community regarding how to deal with the patients and the communication methods.

One more possible interpretation.

Another point I found interesting in this story is that regencies seem to be setting up a chain of command, from different agency heads to community health centers and so on.  It seems a little late in the game to be doing this, but better late than never.

symptoms or conditions in order to get treatment at the bird flu facility, due to panic and rumors.
