the same sentence we have been reading at the end of every BF story for months: the "not if, but when" disclaimer.  The one worrisome aspect of his comment was when he said that it had crossed the species barrier. Pigs, expired tamiflu and resistant virus???
If I am reading this article correctly, then they seem to be saying there is a triple threat in Egypt.

First

This sounds like they are saying that the tamiflu they have on hand is expired, and for that reason (or perhaps viral resistance?) the drug is failing to help cure the disease.

Of course if this is true, then the good news is that the negatives are probably real negatives rather than positives which are masked by tamiflu. The bad news is that if the disease has become tamiflu resistant (as was suspected/discovered by NAMRU3 last summer, I believe) then it is going to be harder to smother this current outbreak.

The other concern is this part about the pigs...

That sounds almost as if the recommendation is to take people within an outbreak area to a quarantine camp or zone in the desert....

Expired Tamiflu
Like most if not all other medicines, Tamiflu's efficacy would degrade somewhat with time.  By and large, though, I consider expired Tamiflu to be the next best thing to Tamiflu that hasn't expired.  If I just bought some that was expired, I wouldn't be happy about it, but I wouldn't be throwing it away, either.

I'd also still expect it to work at least almost as well as it would have before it expired.

that Tamiflu, used beyond its expiration date (and we would not know if it had been stored in the most optimal conditions) would push the virus in some way to further resistance, in the way that administration of inadequate amounts of Tamiflu might?

Expired Tamiflu
Perhaps I shouldn't bring this up because I was unable to find the reference but there was an article that came out of Australia (Or perhaps NZ) sometime last summer (if I remember the timing correctly) that stated that the Tamiflu they had in stock was soon to begin expiring and they had performed tests that showed it was good, with no loss of effectiveness, for at least two years past the date.

That was, again IIRC, heavily caveated with proper storage to begin with.

Anyway, my memory is not always reliable for "bits and pieces" but I decided to "toss it out there" in the hopes that someone would either remember reading it or would be able to track it down.

to mean: they need to cull the pigs which are carriers and immediately transport all the (now dead) pigs which were in the pens out to the desert (I assume to bury or burn them).

Since we don't know:

1) whether the virus is sensitive or resistant
2) whether the conventional doses are sufficient
3) whether they are using conventional or increased doses
4) whether the doses are being given early enough and for long enough

We already have lots of challenges trying to determine what the effects of tamiflu are on the outcomes of patients as well as transmission, before we even get into the expiry issue.  Hence there isn't anything unfortunately to guide us as to how much importance we should place on the expiry issue, and especially I want to point out that it's a bit of a stretch (in interpretation, nothing personal here, ok?) to use this as a possible sign of encouragement, that the negatives are probably true negatives, ie I wouldn't draw such an inference based on the expiry issue at all.

I would agree with Clawdia that expired tamiflu is probably the next best thing to unexpired tamiflu, on the grand scheme of things.  I am at this point assuming that the tamiflu they are giving out are close enough in effectiveness to the real thing, given the range of uncertainties with regards to efficacy (listed above).

I am trying not to be sensitive, but....
It really seems like you jump on and negate my ideas out of hand to a much more frequent and greater degree than you do most anyone else. For example, in the above I'm merely exploring the meaning behind an actual news posting which seems to indicate Egypt's tamiflu might be expired, and you bring up a bunch of other points not really relevant to the particular issue  which is expired tamiflu. You then discredit even the idea of discussing the potential effects of expired tamiflu, saying there are lots of other challenges regarding the effects of tamiflu that are more important/should be explored before worrying about effects of its expiration. Great,maybe so...but the news article was talking about possibly expired tamiflu so that's what I was discussing. If it had been about the other things you mention, maybe I would have been exploring those ideas, but it wasn't so why should I.  As for my looking at this as a potentially encouraging sign, so what? I am not writing a thesis here, I am not publishing a research paper: I'm just looking at possibilities, and that was a possibility. You never never jump on anyone else in this manner and it has become quite hard not to take it personal, regardless of you saying I shouldn't or adding a smiley face at the end of your barbs.

I wasn't jumping on you or anything.  I wasn't even negating or disagreeing with your thoughts.  I was only saying how there are many uncertainties so it's hard to say.

FWIW, I didn't (and don't) "discredit the idea of discussing the potential effects of expired tamiflu", because I do think it is a legitimate question, which was the reason why I posted a response.  What I meant, on the tamiflu issue, was from the information available, I'd tend to be a bit more conservative in drawing inferences, that's all.

I do agree with hindsight that recently you and I seem to be discussing quite a bit on virology and stuff, but that REALLY is only because you often ask really good questions or raise very pertinent points that are of interest to me.  ;-)  I would feel bad if you take it any other way, because I really DO think that you often ask great questions, even if we don't come to similar conclusions about them.  And I would hate for either one of us to think maybe there's something wrong when there isn't.  

We have a lot of great people on the forum, and depending on their interests some people are more often drawn to similar subjects/issues than others.  So apologies again if it came across in other ways than what I intended.  ;-)
