NEW CASE - NY GL - Central java

Hi Mojo
I found a site you may be interested in.  I wanted to find more info on the new case bgw inMT found late yesterday, so I googled "RSU Pandan Arang Boyolali".  That's the hospital.  First thing on the list was this site - with the "new" patient.  Good news about this site:  it has a search feature  :-).  Bad news:  It runs slowly.
If you would like it, let me know and I'll add it to your list.  If not, I'll keep an eye on it.

the search feature is useless.  Nothing in order, according to date.  And this particular patient was not the first thing on the list at google,  it was something from 2006, but this new case was the second article down under "berita" to the left, which was current articles.  This site is very very slow, and I wouldn't give it to you because of that.  But it's up to you if you want to take it.

To get a previous index, click "Cari" at the left, by the search box. This takes you to a previous index. Down at the bottom of this page you can go back to more previous days by clicking on the date.

(Finally a doctor, Dr. Hadi Yusuf confirms what we have suspected all along.  It is not possible for infants this young to contract H5N1 from poultry.  He surmises if an infant has contracted H5N1, it would be from contact with others.)

Babies and H5N1
In some countries, women, while working and accomplishing all home tasks, carry their babies (all day long) on their bosoms, in other countries on their backs.

How do we know if a mother didn't pluck/disembowel/prepare a dead chicken while having the baby on her breast?

Also, many mothers giving their milk to their babies sleep with them in their arms, in any part of the world. Or take crying babies from their cots and soothe them in their beds until they fall asleep again. I recall that some Indonesian families hid chickens under their beds to avoid them being culled.

I would not dismiss these possibilities in assessing at what age a baby could catch bird flu...

Pregnant and H5N1
And what about a lightly clothed heavily pregnant women - what happens to the unborn baby if they catch BF? There was one case I think in Indo where they both died, no?

Is there some issue to explore here? If the pregnancy is far advanced, would it be worth trying to get the baby out to save it even if nothing can be done for the mother?

Should we start a thread to think about this issue?

I wouldn't think it would be advisable to try to save the baby - in a pandemic situation in which nothing could be done to save the mother, the baby, even if born alive, probably wouldn't stand a chance.

Perhaps I'm just feeling a bit fatalistic today.  :(

a pandemic yet. There's still room to try and save unborn babies in Indo and Egypt if such cases arise now.

FG - I thought you meant being in the midst of pandemic with people dying all around and under those circumstances trying to save a baby whose mother was dying of H5N1.

Now, well, I don't know if these places like Indo and Egypt are equipped to go to extraordinary lengths to save babies that in many instances would be premature.

At what point do you take the child,

  At what point to you take the child?

  When the mom can no longer support herself and child? Ok how is that determined? Do you need 35th weeks of pregnacy, 32, 30? Do you give drugs to stimulate lung development for a few days?

  Can you test the child for AI antibodies? (has the virus passed through the placenta?)

  Can the mom breast feed or will she pass AI in the milk?

  Is the child kept isolated after birth?

  When the mom believes she will not make it and asks for the child to be taken?

  Having the father around - does that influence things? probably not, men have few rights as "its a womans body" (quoting here not passing judgement)

  Does hospital environment count? My guess is no.

In the 1918 pandemic, . . .
...the CFR for pregnant women who caught the flu was very high.  AFAIR, around 70%.  Seems the stress of pregnancy combined with the stress of flu is overwhelming

Interestingly, my father was born Sept. 12, 1918, in the midst of the pandemic heating up.  Unbelievable, how lucky he and my grandmother were to have avoided the flu, or lived through the infection...I don't know which.  They lived on a rural farm, and were quite self-sufficient, so that might have had something to do with it.

So, the trick w/pregnancy is to avoid the flu altogether.

And we really don't know what effect the health authorities are having in trying to educate the public about bird flu. It seems, however, from some of the more recent stories that families are much more concerned than they were earlier, and I hope that would mean they are sheltering the children from possible infection.  But again, we are just speculating.

YOGYA (KR) - the Patient who was expected suspect bird flu might leave Isolation Space of RSUP Dr Sardjito after results of the fast test of Balitbangkes Jakarta stated the negative.W (42) from Potorono, Banguntapan, Bantul was stated by the bird flu negative, so as to be able to come home since last Thursday (19/4).  Therefore up until yesterday in RSUP Dr Sardjito already did not have again the patient suspect bird flu.

  "Hasil rapid test from Balitbangkes Jakarta said that was relevant the negative so as to have been permitted to come home, the condition for his health has been restored, might not be very old in RSUP Dr Sardjito," the legal headword and public relations of RSUP Dr Sardjito Drs Trisno Heru Nugroho MKes, when it was confirmed about the patient suspect bird flu that was reconciled in RSUP Dr Sardjito several previous days.  According to him, before the patient W came home on last Thursday, the patient suspect bird flu beforehand also has been permitted to come home because of being proven to be negative.

I read this as the blood work was near normal but the X-ray of the lungs and the symptoms the patient is experiencing is leading them to believe he does have bird flu. Could it mean they are finally waking up to what we suspect has been many cases of false negatives? I sure hope so.

The patient's name should be Maksum NOT Mustafa....That's what I get for trying to do this when little brain is on overload.
Mustafa's chicken's just died. So far as I know he is not sick.

Cases in Java seem to be spreading since January.
First there were large outbreaks in West Java, then East Java, back to West and now central. Are there bird outbreaks that correspond? If not, what might be causing these clusters to jump area to area and back and forth, rather than slow geographical spread. Sounds like H2H, from the pattern.

NOTE:I think this is an important article. Evidently Malik Peiris is going to Suraybaya to help Airlangga University (Unair) with some research. It sounds like he is going to have to go in the market himself to do it. I would have thought that was unnecessary but Indonesia sounds like they are playing diva again. While they have the whip hand they are going to use it, I guess. They refuse to sign something or other until later. They make it sound like they are doing Malik Peiris the favor by letting him work in their country. Maybe my interpretation is wrong. I have put translations for the untranslated words in {...}. I hope they help and don't hinder. Maybe SusanC will be able to read more into it.

they are going to do epidemiological studies in the markets.  One of the gaps in scientists' understanding of what is happening in Indonesia has been the lack of data on avian sequences.  Sampling in the wet markets is a key activity which will tell us a lot about how the virus evolves. 

This type of research has been done for a number of years in the university of Hong Kong, primarily on samples from the markets of southern China.  The 'discovery' of the Fujian-like dominant strain was the result of such a survey.

It sounds like he is going to have to go in the market himself to do it. I would have thought that was unnecessary but Indonesia sounds like they are playing diva again.

Actually, it is very important that Malik gets first hand knowledge of the conditions in Indonesian markets and their practices in detail.  Epidemiology is not a dry and routine science.  It's a lot of detective work.  You are much more likely to find vital clues if the-guy-who-knows-what-questions-to-ask goes to the crime scene himself!

They refuse to sign something or other until later. They make it sound like they are doing Malik Peiris the favor by letting him work in their country.

I can't figure out what that might be, but this is followed by comments on joint publication of research, so it may refer to some arrangements for data sharing or funding, maybe.  Just guessing.

They are doing each other, and the world, a favor.  ;-)

leads us to make wrong inferences. Now that you mention it I can see the importance of him being actually in the market. Surely he doesn't have to swab duck bottoms though, I hope.

same agreement to do cooperative research. Maybe they just don't know he's one of the world's top experts in the field. I hope he gets access to some samples that they can research and sequence in Hong Kong. I would still like to know what "kesepahaman" means.

Are there any fluwiki archives of statistical data on cases?
I know fluwiki has been tracking cases for some time now, and I was just wondering if anyone kept an archive file anywhere that listed the suspect cases etc last year, so we could compare them to this years cases.

I recall asking previously about any kind of statistics related to how many flu cases might Indonesia be normally expected to have, and of these what percentage might be expected to turn into pneumonia.

What I am trying to figure out, one way or the other, is the lingering, nagging question we all have asked time and again: are the "negatives" truly negative?  Is it possible/likely that there could be this many seriously ill with secondary complications from ordinary flu, including pneumonia, low thrombocytes and leucocytes, the need for ventilators and intensive care treatment. Is this number of complications within the normal, expected range in a given flu season?

Always have a plan B.

The maternal mortality and illiteracy rates in East Java show that women have been neglected by development, activists said Friday.  In a demonstration held to observe Kartini Day, which falls on Saturday, activists from the Women's Pro-Democracy Committee urged the government to set up a system to ensure women were not considered second-class citizens.

"In our records, the maternal mortality rate in East Java is 364 per 100,000 births," said committee leader Erma Susanti at the protest outside the Grahadi building in Surabaya, East Java.

The 2005 World Health Report put the country's maternal mortality rate at 307 per 100,000 deliveries. The protesters also focused on the lack of a special budget to empower women, such as through improving mothers' health care. The absence of such a budget, Erma said, was responsible for the country's high number of illiterate women.

In East Java alone, 2.8 million had not been educated, she said. The committee also recorded that 17.4 percent of the province's 35 million residents were illiterate women.
Across the country, there are around 15 million illiterate men and women, they said.

I'm working on a little stand-alone applet that will let us translate ten (or maybe a few more) words at a time when Toggletext is down.  There'll be a place to paste the sentence, and it'll fill in the English.  You'll be able to see ten words on the screen (or more if I can fit them on), and you can choose which definition you want, and drag it to a blank field below, where you can play with the word order, and copy it to paste into Word or Notepad or whatever.  So, something like:

if you remember the rest of that, you're almost as old as I am! 

I should be able to put the first version out there by next weekend.  I probably won't be able to get all the words in by then, but I'll put what I have, and add more as I can.  I'm getting the list of Indonesian words from http://www.seasite.n... and I'm putting the prefix+base or base+suffix in as separate words.  That's one of the hardest things about looking them up in a dictionary; trying to figure out what the base word is.

There are some free Indonesian dictionarys in software
form that you can download from the Babylon site. I found three of them. One of them has 60,00 plus words. They are done by volunteer individuals so I presume they don't belong to Babylon. I have no idea about computer programming, so I don't know whether you could make any use of these or not. They are not stand-alone applications as far as I know.

The one called Balifolder was the biggest I believe. The names of the other two are Indonesian English Online and __Mas_NDon_Indonesian_to_Engli.BGL.

Mojo, I have a better site for you than the one that is slow as molasses.
That one is not just slow, it's just about nonfunctional. My email is in my profile, or write a message here if you want it. I'd just drop the other one if I were you. I found a whole bunch of sites like it that are plenty fast. I think these kind of sites, that end in .go.id are sites of the governments of the different regencies, or sub-districts, or what ever they call that level of government. They still have some good articles sometimes.

If anyone else wants a site let me know. I have been trialling several lately. I have a whole bunch more than I need. I don't keep them unless they have a workable search feature or a good index (or better yet, both),or I find several stories that make them worth being a pain in the neck.

I told you I had found several sites of this type that were a lot better than the other site you were going to take because they loaded in a reasonable amount of time. I had looked through them and they seemed to be OK. Tonight, I went to my bookmarks file to get the links that I had made last night, and they weren't there! The browser program must have eaten a worm or something. LOL

Anyway I got busy and found a bunch more just like them, but I haven't had time to check them out. The first site I know all about because I have been checking it every day for the last two months. If you want it, you are welcome to it. As I said, I've got too many lined up anyway (not including these). Except for the first site, the links below are to new sites that aren't on anyone's list. I pulled them up once to see if they looked good, but did not check the search function or check to see if they had an index (sometimes called an indeks, arsip or arsip berita). Anybody that wants one of these is welcome to them but I'd advise checking them out before you sign up for one.

Kelly, I worry about that Tribun Batam site you took. It may not
be productive enough to warrant fooling with it. I don't want to just waste your time. I've never found a search or an index on it and I just got through looking at it again. Since you took all those extra sites, too, you probably would be glad to just drop it.

It doesn't mention testing any of the other kids in the biology class?
I am a bio teacher. If we do dissections, it generally is in small groups working together as teams on a single specimen. I suppose all the students might have had their own chickens - especially if they are plentiful - but my first thought was about the other kids in the team being exposed. Also, where did they get the chickens they dissected? The article mentions doing an inspection of the chickens around the house of the victim, so does that mean he brought the chicken he dissected from home? If all the students brought their chickens to dissect from home, have they checked the chickens from all of the students' homes? There could be cross contamination, an exchange of chicken parts during the dissection or other possibilities that don't seem to be explored here,.

My Babylon trial expired today and won't work anymore. I tried to buy it yesterday four different times but couldn't make the payment process go through (and believe me there's nothing wrong with my credit cards).

The Babylon company has rather agressive marketing techniques.  If you are interested in this software, do not expect text translation. It does that in some of the other languages (including Arabic and Farsi), but for Indonesian you have to download separate dictionaries and it only translates one word at a time.

A dissection is done on a fresh or preserved specimen. In this case obviously fresh, as he took the remains home later and ate them (which is probably what entok means...carcass or something.) The point is, when you do a dissection you are cutting into the animal, pulling out and examining all the various organs. There is plenty of blood, plenty of chance for infection. Very often a dissection in a school lab - especially if there is not alot of money - does not include the use of sterile gloves, masks or eye wear. So if this chicken is the source of the child's infection, what about the exposure of his school mates? And where did the chicken come from in the first place? Are we sure it was his chicken that was the source, or could he have gone over during the lab and helped dissect one of his class mate's chickens? All sorts of possibilities for exposure in this classroom activity, and I am asking if any of the other kids and/or their home poultry has been checked for H5N1.

I have run into a dilemna in updating the case table.
In backtracking to ensure our positive count is in line with the Indonesia official count, there are several individuals who have been reported "positive" by the media, but who have never been acknowledged as positive by the Indonesian officials or the WHO.

If no one objects, I'm going to add another line to the table with the label "Media reported positive."  I think some possibly relevant information will be lost if these reports are simply moved back to the "Symptoms, tests pending" category.

Information Diary has the numbers of positive cases since WHO stopped reporting and only includes those cases officially announced by the Indonesia Department of Health.

You will note in the Case Information for April that there is a case that was reported positive by the media but was never confirmed by the DoH.  Later, the testing hospital stated that they are still seeking approval from the DoH to have their test results recognised.

So, the numbers that appear there, and in the spreadsheets you would have got from Carol and Floridagirl, are in accordance with the Indonesian officials.

Casualties BIRD FLU improved AGAIN two ORANG

Jakarta, 30/3/2007 (Kominfo Newsroom) - case casualties Avian influenza (Burung= FB Flu) in Indonesia improved again two people after Y (P, 28 years) from the Serdang District, Jakarta the Centre and D (L, 14 years) from the Banyan Tree Cape, West Sumatra, died although both of them could be treated in the hospital.

S. Sulistyowati, MM, that was met in his office, on Friday (30/3), said with the death of the two people then till March 29 2007, the positive FB case in Indonesia became 91 cases, 71 among them died.
"Angka the Death or Case Fatality Rate (CFR)  reached 78.02 persen," said Dr. Lily.
He also said that Y and D were stated positive terjangkit the virus H5N1 through the inspection PCR the Body of Health Research And Development (Litbangkes) the Department of Health on March 28 2007 and the Laboratory of Eijkman Jakarta on March 29 2007.

I'll have to look into D (14) later today.  I may have thought this D was Didi who was declared positive a few days earlier.

I don't want any reported positives to go untracked, so I'll do some more researching.

actually, I wasn't checking. I saw an older article....
and thought it might give us some information on this "level 5".....and I saw the information on "D"....

I researched and now remember
why I didn't show D (14) as a separate person.  I re-read AnnieB's research and comments solicited by FloridaGirl and feel that D(14) is likely the same as Dermanto.  There was lots of discussion on whether Dermanto's age was 40 or 14 and if Desmanto was another name used.

Since the count was consecutive with Y's count, I think this is the right answer (based on the limited information we have).
