Free medical treatment in East Jakarta
BeritaJakarta.com East Jakarta Indonesia Red Cross (PMI) holds free medical treatment on Jl I Gusti Ngurah Rai 77, Klender, East Jakarta, Wednesday (3/10). According East Jakarta Head of PMI, Kusnoto, the program is held due to help the society during fasting month. "The society is very happy on this program and they ask to hold the same program again," said Kusnoto.

Kusnoto stated that East Jakarta PMI also opens Griya Medika PMI Public Clinic that will serve the society such as medical treatment with cheaper tariff. The clinic will be opened on Monday up to Friday at 08.00-12.00 WIB and continued at 15.30-20.00 WIB. "The tariff is only Rp 15,000," stated Kusnoto.

Not only opening clinic, East Jakarta PMI is also giving free ambulance 24 hours service for those who can not pay the ambulance service. "The program is followed by 300 patients," finished Kusnoto.

Abuse of Tamiflu can create resistant bird-flu strains

Scientists led by Jerker Fick, a chemist at Umea University, tested the survivability of the Tamiflu molecule in water drawn from three phases in a typical sewage system.

The first was raw sewage water; the second was water that had been filtered and treated with chemicals; the third was water from "activated sludge," in which microbes are used to digest waste material.

Tamiflu's active ingredient survived all three processes, which means that it is released in the wastewater leaving the plant.

The finding is important because of the risk that Tami­flu, if overprescribed, could end up in the wild in concentrations high enough to let H5N1 adapt to this key drug, the authors say.

Flu viruses are common among waterfowl, especially dabbling ducks such as mallards, which often forage for food near sewage outlets.

"The biggest threat is that resistance will become common among low pathogenic influenza viruses carried by wild ducks," said co-author Bjoern Olsen, professor of infectious diseases at the University of Uppsala and University of Kalmar.

These avian viruses could then recombinate with ordinary human flu viruses, creating new strains that are resistant to Tamiflu, he said. http://www.manilatim...

(I am only posting this here for the purpose of commenting that this may be one of the problems we will be seeing in Indonesia soon. They are distributing large amounts of Tamiflu and the sewage systems and waste water treatment plants are substandard. I am very familiar with this process. Residental, commercial and industrial sewage is sent to wastewater treatment plants. It is put through a series of filtrations and is chemically treated. The final product of "activated sludge" is compressed and dumped back into the enviroment as dirt for land fill, bagged and sold as potting soil, etc. The filtered and treated water is sent back out as potable water and goes back to homes and businesses. It is the same as what comes out of your faucet everyday!)

I don't think they put the water back into the system
At least in Los Angeles they don't. As I teacher, I did several tours of the wastewater treatment plant. The treated water is used for watering public area lawns and plantings of a non edible nature (govt buildings, parks etc.) as well as used by some golf courses. Even though it is totally purified and safe, they just know people will object, and if anyone got sick - even though its cause was NOT the treated water - they would surely sue the wastewater plants.

In places like Indonesia, however - especially in the small towns and villages - I doubt that they even have wastewater treatment. More likely villagers have individual septic or cesspool type disposal for their sewage at best. With lots of rain, the untreated contents make their way into the groundwater and nearby surface waters fairly rapidly, i'm sure.

Always have a plan B.

I was just trying to give a shortened version of the process instead of putting in every detail, which I still am not going to do because it would just bore everyone. I designed systems & sold equipment & chemicals for part of the operation of wastewater treatment plants. I worked in the water treatment industry for over 20 years.

The filtered and treated water from the wastewater treatment plants is sent out to the rivers, streams, reservoirs, etc. Water from these sources is collected and  goes  to a separate type of treatment plant where it is filtered, chemically treated and then comes out of your faucet. Btw, ground water and runoff, including water used for lawns, eventually returns to rivers, streams and reservoirs. It is all one huge cycle.

I was in a hurry when I wrote this and it wasn't meant to be "snippy". You are right about the poor sanitation in Indonesia. There are wastewater treatment plants in the cities there but probably not up to US, European, etc., standards. In the rural areas the population mostly relies on well and river water. The concern is in over prescribing Tamiflu. Since it doesn't break down in the chemical waste treatment process and it doesn't break down in the environment, the results could be a Tamiflu resistant H5N1 strain. This was seen in Egypt but I don't know what caused it. Theresa42 is very knowledgeable on the facts in the Egypt cases. The point of all this worry is a resistant strain could happen anywhere Tamiflu builds up in the waterways. With trying to keep bird flu under control, I personally don't see how they can keep from blanketing areas with it. It is a scenario of "damned if you do and damned if you don't.

Red Cross Cooperates To Prevent Pandemic

04 / 10 / 2007 20 : 22 : 29 Bantul (KR)- Indonesia's Red Cross Chapter (PMI) in Bantul has cooperated with the International Red Cross Federation to maintain a Psikososial (Psychological?) Support Program in County Bantul Province for earthquake victims to reform psikososial (psychological) condition frame, because society is impacted by earth quake disaster of May 27, 2006.  According to Team Volunteer Coordinator/Profiler PSP PMI Bantul Chapter, these activities have been implemented in town for some time since and will be coming to an end soon in December 2007.  The Psikososial Program supporting this takes place in two communities, society and school, and uses participative methods.  The Team cooperates with society to surmise community necessities, to activate the return of activities in society or schools, and to espouse psikososial reform process and to improve society's well being.

This time PMI Bantul Chapter Team Volunteers assisted society and arrived soon with boots at the end of the year 2007, in the hamlets of Pelemadu Sriharjo Imogiri, Pandeyan Sitimulyo Piyungan, SD Plebengan, SD Barongan and Teacher Association TK a Selopamioro Imogiri.  While Bantul PMI Chapter has now also formed a Bantul PMI Avian Influenza Handling Team.  According to Bantul PMI Chapter Budianto AMd Secretary to KR, this formation followed after run by Semiloka Bantul Chapter PMI Avian Influenza with Taruna Disaster Preparedness (Tagana Bantul) a few days ago in Hotel Adinda Parangtritis, that had participants from Bantul PMI Volunteer Chapter, Karang Taruna and Bantul County Tagana.  This Avian Influenza Handling Team consists of a Task Force Team that has 5 academic members and 20 volunteer members.  The target group includes families, mothers, toddlers under the age of five, school aged children, chicken merchants, livestock owners, society in general and PMI volunteers.

The impressive thing about the Semiloka organization and the formation of this PMI Avian Influenza PMI Handling Team PMI is because it formed a humanity organization to executive their perspective to prescribe quick, accurate and effective help to society. The Bantul PMI Chapter works to have an active role in handling a bird flu epidemic so that it does not happen especially in DIY Bantul Province.

Feared bird flu death in Indonesia (Ln ?)

An Indonesian woman suspected of being infected with bird flu died today on the island of Sumatra, a hospital official there said.

Blood and tissue samples from the 44-year-old victim have been sent for testing in Jakarta, said Azizman Daad, a doctor at the Arifin Achmad Hospital in Riau province said.

Two tests must come back positive for the H5N1 virus before a victim is confirmed as part of the official bird flu death toll in Indonesia, which is the highest in the world at 86.

If confirmed, the latest victim would be the nation's 87th fatality.

"The woman who died early this morning was showing symptoms of bird flu infection, such high fever, coughing and breathing difficulties," he told AFP.

Daad said it was unclear whether the woman, who lived in a residential complex run by US oil company Caltex, had been in contact with poultry.

H5N1 is endemic in birds across nearly all of Indonesia.

PEKANBARU (Antara): A suspected bird flu patient identified as LT, 44, died at Arifin Achmad Regional General Hospital on Friday, official said.

"The patient was treated in an isolation room for bird flu patients at Arifin Achmad Regional General Hospital," Burhanudin Agung of the Riau provincial health office said.

The resident of PT Chevron Pacific Indonesia (CPI) housing complex at Rumbai, Pekanbaru, had been treated at the Awal Bros hospital for four days before he was transferred to the Arifin Achmad Hospital on Thursday following his worsening fever.

The hospital reported his blood sample had been sent to the Health Ministry's laboratory in Jakarta to check whether he had died of the bird flu virus.

The hospital also sent a blood sample of another suspected bird flu patient identified as five-year-old RS, a resident of Risak Hulu, Kampar District, Riau Province.

Riau has recorded four bird flu cases with three casualties since 2005. Indonesia is the hardest-hit country, with the virus has claimed lives of 86 out of 107 infected people as of Monday. (**)

Pekanbaru, Riau (ANTARA News) - A suspected bird flu (Avian Influenza/AI) patient identified as LT (44) died at Arifin Achmad Regional General Hospital here on Friday.

"The patient was treated in an isolation room for bird flu patients at Arifin Achmad Regional General Hospital," Burhanudin Agung of the Riau provincial health office said.

LT was a resident of PT Chevron Pacific Indonesia (CPI) housing complex at Rumbai, Pekanbaru.

Before being transferred to Arifin Achmad Hospital, he was hospitalized at Awal Bros Hospital in Pekanbaru for four days. But because his fever continued, he was moved to Arifin Achmad Hospital on Oct. 4, 2007.

However, doctors at Arifin Achmad Hospital could not confirm whether LT had been infected with the avian influenza (bird flu) virus and therefore he was still considered a suspected bird flu patient.

A sample of the dead suspected bird flu patient`s blood was sent to the Health Ministry`s laboratory in Jakarta to check whether or not he had died of the bird flu virus.

In addition to LT`s blood sample, the hospital had also sent a blood sample of another suspected bird flu patient identified as RS (5), an inhabitant of Risak Hulu, Kampar District, Riau Province.

From 2005 to 2007, there have been 27 suspected bird flu patients in Riau. Of the number, four had been confirmed as infected with the bird flu virus. And of the four bird flu patients, three had died and one survived.

Meanwhile, the Indonesian Health Ministry said in Jakarta last October 1, 2007, that the total number of people so far affected by the virus in Indonesia was 107 and the number of deaths by the disease was 86,

In Jakarta alone, the number of bird flu cases has been recorded at 26 of which 23 have ended in the sufferers` death. (*)

It would be nice to see a flood of news on the native Indonesians who have died of bird flu but most of the time you newshounds have had to dig out every bit of information. When they say "camp" do they imply that these work locations
include housing for the workers, like small self contained cities?  If so, in a sense they are kind of like army camps. This might make for close living conditions and interaction among the workers and families, helping to spread the virus among them. On the other hand, it might be easier to contain the virus to this area if they do a quick quarantine. I would be looking for news on that in the near future.

3 of the past 4 suspected cases since 9/3, have all gone there before going to RSUD Arifin Ahmad.  The only one who hasn't would be RS (5).

Fears over risk of exposure to bird flu virus increase in Indonesia as the country prepares to celebrate Idul Fitri to mark the end of the fasting month, warned the UN's Food and Agriculture Organisation (FAO).

A huge number of poultry and people will move across the country during this festive period, and the festivities, which mark the end of the fasting month, will involve a lot of food, with poultry topping the list.

"There's a tremendous amount of movement of people and poultry. And with that movement there's an increased risk for both people and for ducks and chickens to come into contact with avian influenza virus," said Dr Eric Brum, chief technical advisor of FAO.

Indonesia has the world's highest mortality rate from bird flu. Of the 107 infected with the deadly H5N1 virus, 86 have died. The virus has spread to all but two of the country's 33 provinces.

Organisations such as the FAO and the World Health Organisation (WHO) are deeply concerned.

"I think because of this rare disease? we need to take appropriate precautions so that we don't play with fire and this game of chance. We're working very closely with the Indonesian government, the ministry of health especially, in trying to communicate this message to the public," said Dr Vernon Lee, WHO Indonesia's avian influenza epidemiologist.

Some 1,200 officials will be part of the monitoring and response teams spread across Indonesia.

People will be advised to take precautions, including keeping their chickens in quarantine for two weeks before slaughtering them to minimise the risk of infection.

As commercial poultry farms have to adhere to strict hygiene protocols, it is the free-roaming backyard fowl that authorities are worried about.

There are an estimated 300 million free-roaming chickens in Indonesia.

Endemic bf throughout Indonesia. 300 million free roaming chickens. World's most populous country with 234 million people spread throughout 17,508 islands. A huge number of poultry and people traveling across the country during this festive period. Only 1200 officials assigned to monitoring. Warning from the FAO stated as "we need to take appropriate precautions so that we don't play with fire and this game of chance". This doesn't sound good and I don't like the odds!

The HA sequence of H5N1 in swine in Indonesia has been made public.  It is the first public H5N1 sequence from swine outside of China.  The isolate, A/Swine/Timka/BBVM186B/2007(H5N1), has typical Indonesian markers and is clearly clade 2.1.  (snip)

A notable acquisition in the HA sequence is M230I.  This acquisition is also in recent chicken isolates in Papua as well as West Java.  Therefore it is widespread, but has not yet been reported in human H5N1 from Indonesia. However, the most recent public human sequences from Indonesia were human cases in early January.

The presence of M230I in mammals is cause for concern.  M230I is present in all three circulating strains of seasonal flu (H1N1, H3N2, influenza B), and has been seen in human H5N1 cases in Egypt (the Gharbiya cluster, which is the largest cluster to reported to date in Egypt).

One of the worst cases of flu I ever had started 2 days after I returned from an out of town funeral. It seems at a time like that there is even more body contact (hugging and wispering in one's ear) than at any other time and event. I don't know the distance from where she lived to the area of the funeral but since it was a different island, I was thinking there would be less exposure to other people while traveling if it had been done by car/ferry than the close quarters of a packed plane. I hope they don't just stop collecting samples when they finish with her husband, kids and neighbors. I hope they check every contact at the funeral and every contact they can find that she encountered in travel. I think we need to watch this one closely to see if anything developes and hopefully it won't!

Another concern is how long they waited to treat with tamiflu.
It seems like they are still following the protocol of "no chickens, no treatment" until it is too late to help. As the virus gets more prevalent in other hosts such as cats and pigs, one would hope they might update their protocols accordingly.

PEKANBARU, Indonesia, Oct. 5 (UPI) -- "Health officials in the Indonesian province of Riau said they suspect that a man who died there Friday had been infected with the bird flu virus. Riau provincial health office official Burhandudin Agung told the Indonesian news agency Antara that the patient, identified only as LT, had been treated as a potential victim of the virus until his death Friday.

"The patient was treated in an isolation room for bird flu patients at Arifin Achmad Regional General Hospital," Agung said.

Doctors at the hospital have been unable to prove that LT had been infected with the potentially deadly avian influenza virus. A test sample of his blood was sent to the Indonesia Health Ministry following his death to determine whether the virus had caused his death."

Pekanbaru (Lampost) : Again a suspect bird flu patient died in the world in RSUD Arifin Achmad Pekanbaru.  The patient Linda (44), a family mother Perumahan PT Chevron resident in Kecamatan Rumbai, Pekanbaru.  Within three recent months three deaths from bird flu have already been recorded.

I am betting that everyone who comes in with fever and anything even related to flu symptoms will get Tamiflu. It is my understanding December is the busiest month of the year for tourist to come to Indonesia and so they will be extra cautious right now. Also watch for suspected cases of bf to be even more hushed-up than usual (if that is possible) between now and the end of the peak tourist season.

Due to the possibility of terrorist attacks directed against American or other Western citizens and interests, the Department of State urges American citizens to evaluate carefully the risks of travel to Indonesia. The October 1, 2005, terrorist attacks in Bali, in which suicide bombers killed 20 people and injured more than 100, are a reminder that terrorists remain active in Indonesia. Similarly, during the three prior years, there were three significant terrorist attacks in Indonesia, two in Jakarta and one in Bali. During 2007, the Indonesian police and security forces disrupted a number of cells linked with Jemaah Islamiyah, a U.S. Department of State-designated foreign terrorist organization. The existence of additional cells intending to carry out future attacks in Bali, Jakarta, or other areas of Indonesia cannot be ruled out.

Terrorist attacks in Indonesia could occur at any time and could be directed against any location, including those frequented by foreigners, as well as identifiably American or other Western facilities or businesses in Indonesia. Such targets could include but are not limited to places where Americans and other Westerners live, congregate, work, study, shop, or visit, including hotels, clubs, restaurants, shopping centers, identifiably Western businesses, housing compounds, transportation systems, places of worship, schools, or public recreation events. I find this inexcusable

With all the expansion of the H5N1 virus throughout Indonesia, with the deaths recorded since this fact sheet was released and with knowledge gained over the past 15 months, I find it inexcusable our State Dept. has not updated the fact sheet since July, 2006. They are saying the danger of bird flu in Indonesia is the same as it was then. That is simply not true!

Why would the planners for this enormous conference on Global Climate Change
which is drawing thousands of high level diplomats and scientists from around the world, choose to have it in a place KNOWN to have terrorist groups that are totally opposed to western powers?  If I were a terrorist, I'd be grinning ear to ear right now. They're going to put 7000 of my "enemies" into a single confined space, in a remote country where the government can't control backyard chicken populations let alone offer protection to a huge group of officials from around the world. Talk about sitting ducks!

Add to those very real dangers the issue of bird flu and other diseases rampant in the area, and one just has to wonder how smart these smartest people in the world really are.  Intelligent? Sure. Book smart? you bet. Street smart? Uh, nooo, it would appear not.

And it's not even the idea of having their conference in a place with beautiful weather to offset the potential dangers, as they've picked the rainy season for it. Obviously the conference planners are getting some kind of kick backs from Bali. But why would the delegates to this conference not question the choice?

Am I just too pessimistic, or what?

Commonground and I searched and searched yesterday for anything on this complex and the company. Really nice place. So, my next question is where did she go a few days before she got sick? Did she go to any open markets? That is the only explanation I can think of. If she did not, well the only thing left to assume, is she's been around sick people, not knowing they had BF. H2H2H. I know it's not certain whether or not this woman had bf, but if she is confirmed, then this will be the second case from in/around Pekanbaru that I can recall (and there may be more that I don't recall!) where the person was not a farmer living closely with their poultry or something to that effect.

I don't remember her name (I'm sure commonground will!) -- but there was that Chinese-Indonesian woman who was in business with her family running (or at least working in) a shop selling mobile phones.  Remember, she was transferred from Pekanbaru to that private Gleneagles hospital in Medan?  She lived in a nice, high-rise complex, too.
