Yogyakarta:  Three residents of Bantul, Yogyakarta, a mother and two children, Erna Maryatun, 35, Zaky Wiryawan, 11, and H, 2.5 years old, are in intensive care in Ward Kartika, Dr. Sardjito Yogyakarta.  They are suspected of having bird flu.  Head of Legal and Public Relations Department of Sardjito Yogyakarta, Rini Princess Sunaring, justified the existence of three residents of Bantul, who are a mother and two children, receiving special treatment for bird flu patients.

"In Ward Kartika we are treating three bird flu suspect patients who come from Bantul, Yogyakarta," he said Wednesday, September 7, 2011.

He said the three bird flu suspect patients were referred from one hospital in Bantul, with initial symptoms such as bird flu.  However, to ensure that all three are positive for bird flu, they are still awaiting lab results from Balitbang in Jakarta.  Rini further stated that since the three patients being treated at Dr Sardjito in a special Kartika ward has gradually improved.

"The condition of the patients who were referred and admitted to Ward Kartika in early September, their conditions are getting better," he said.  

Rini added the entire costs of the bird flu suspect patients was free.  

"The bird flu suspect patients care is free at Dr Sardjito," he concluded.

Meanwhile, the Regent of Bantul, Yogyakarta, Sri Suryawidati said that his department has not received reports of Bantul residents who are bird flu suspects.

"I actuallly have not received reports of Bantul residents who are bird flu suspects," he said.

Improving surveillance and scientific research will enable Indonesia to cope with the threat of the avian influenza virus (H5N1), the Health Ministry says.

Health Minister Endang Rahayu Sedyaningsih said on Tuesday that as of Sept. 1, there had been seven confirmed cases of H5N1 infections in humans this year in Indonesia, adding that there had been more cases in birds.

"The avian flu virus tends to cause losses among livestock since it can mutate faster in animals than in humans," Endang told journalists after the inaugural ceremony of a three day table-top exercise on global health security held by the ministry and the World Health Organization (WHO).

Despite the low rate of human infection, she said the disease was still a threat because the virus frequently mutated.

"We should keep on guard against the possibility of a mutation into a new H5N1 virus subtype that could infect humans," Endang said.

To cope with the virus threats, the government is improving the BSL-3 Laboratory at the Health Ministry's Research and Development Unit. The BSL-3 Laboratory has the capacity to contain agents that may cause serious or lethal diseases as a result of contact or inhalation. The laboratory is able to sequence viruses both from animals and humans.

The government is allocating Rp 2 billion (US$232,000) for the development of the laboratory.

"We still need more funds especially for research activities since the allocated budget can cover only the running of the laboratory," Endang said.

Indonesia and other global communities are now facing various challenges related to the spread of various microorganisms that cause emerging communicable diseases. From 2002-2003, Indonesia experienced an outbreak of Severe Acute Respiratory Syndrome (SARS), followed by an H5N1 influenza pandemic in 2009.

The experience in tackling the SARS pandemic led to the adoption of the 2005 International Health Regulation (IHR) at the 58th World Health Assembly (WHA).

It mandated that countries increase their capacity to detect and respond to public health risks to meet international standards.

Keiji Fukuda, the WHO's assistant director general for health security and environment, said pandemic influenza was one of the most important examples of health security dangers. However, he added, there were many other infectious diseases and health risks as well.
He cited the recent tsunami and ensuing radiation threat in Japan as an example.

Fukuda said that all sectors should contribute and work together in a coordinated way to address such health risks due to their huge impacts on health, economy and social communities.

Countries and organizations, he said, should work on planning and be well-prepared, so once something happened, they would know what they were going to do.

"If the next virus is very severe, we will see more illnesses and a pandemic. We should be well-prepared," he said.

Indonesia, he said, had also recently played an important role in the passage of Resolution WHA 64.5 on Preparedness against Influenza Pandemic: Sharing Viruses and Vaccines and other Benefits.

The resolution outlines approaches on how to share viruses and vaccines and other benefits.

BOLMONG: Department of Agriculture and Livestock (Distanak) Bolaang Mongondow (Bolmong) have not found the back of bird flu cases in 12 sub-region after a week of such cases appeared in the Village of South Tanoyan.

Abstract (provisional)

Background
Indonesia is one of the countries most severely affected by H5N1 highly pathogenic avian influenza (HPAI) virus in terms of poultry and human health. However, there is little information on the diversity of H5N1 viruses circulating in backyard farms, where chickens and ducks often intermingle. In this study, H5N1 virus infection occurring in 96 smallholder duck farms in Central Java, Indonesia from 2007-2008 was investigated and the molecular and antigenic characteristics of H5N1 viruses isolated from these farms were analysed.

Results
All 84 characterised viruses belonged to H5N1 clade 2.1 with three virus sublineages being identified: clade 2.1.1 (1), clade 2.1.3 (80), and IDN/6/05-like viruses (3) that did not belong to any of the present clades. All three clades were found in ducks, while only clade 2.1.3 was isolated from chickens. There were no significant amino acid mutations of the hemagglutinin (HA) and neuraminidase (NA) sites of the viruses, including the receptor binding, glycosylation, antigenic and catalytic sites and NA inhibitor targets. All the viruses had polybasic amino acids at the HA cleavage site. No evidence of major antigenic variants was detected. Based on the HA gene, identical virus variants could be found on different farms across the study sites and multiple genetic variants could be isolated from HPAI outbreaks simultaneously or at different time points from single farms. HPAI virus was isolated from both ducks and chickens; however, the proportion of surviving duck cases was considerably higher than in chickens.

Conclusions
The 2.1.3 clade was the most common lineage found in this study. All the viruses had sequence characteristic of HPAI, but negligible variations in other recognized amino acids at the HA and NA proteins which determine virus phenotypes. Multiple genetic variants appeared to be circulating simultaneously within poultry communities. The high proportion of live duck cases compared to chickens over the study period suggests that ducks are more likely to survive infection and they may better suit the role of long-term maintenance host for H5N1. As some viruses were isolated from dead birds, there was no clear correlation between genetic variations and pathogenicity of these viruses.

JAKARTA:  During every event of a pandemic, outbreaks of disease are widespread, and people become victims. Generally, they respond with panic, with limited information, low trust in the government and others, as well as the circulation of unfounded issues.

Kuala Tungkal:  Three Districts in the District of West Tanjung Jabung (Tanjabbar) are bird flu disease-prone.  The three districts are District Betara, High Cliff District, and District Tungkal.  The reason, according to Head of Animal Health Animal Husbandry Department Tanjabbar, drh Sri Rejeki, are that the chicken population in the areas at the community level is high enough, otherwise the chicken traffic selling is also high, compared to other districts.

Forest Ministry Targets Non-Tax Wildlife Revenues

JAKARTA.  Ministry of Forestry is trying to boost non-tax state revenue (non-tax revenues) from commercial plants and wildlife.  The target is Rp 5 billion this year.  Until this September, non-tax revenues from trade in wild plants and animals already reached USD 3.157 billion.  Director General of Forest Protection and Nature Conservation Darori is optimistic the target could be reachedm because the issue of bird flu has subsided.  Darori said that beginning in 2011 almost 100% of bird exports to Europe were rejected because of the bird flu issue.  

"However, mid-year we are convinced the bird flu in Indonesia is not there anymore so they can now be exported," said Darori this weekend.

Darori revealed that currently foreign nationals are very interested in wildlife from Indonesia such as crocodiles, fish, snakes and lizards while the plant of interest is the aloes.  Based on data from the Ministry of Forestry, through September of this year, Indonesia has exported around
34, 233 alligator tails to get non-tax revenues of about Rp 43.2 million.  While fish exports reached as many as 56,248 heads or about Rp 1.47 billion.  For the aloe plant there were as many as 902,904 kilograms of non-tax revenues equal to Rp 1.03 billion.  Darori said that efforts to increase the exports are done with raising captive breeding in some areas.  The one being promoted is breeding snakes.  

"In the near future we will release the new 1,000 hatched snakes in Lamongan, East Java," he said.

Director of Conservation of Biological Diversity (CBD) Bambang Novianto is sure the target non-tax revenues can be achieved.

During the calendar year of 2009, Indonesia failed to report human H5N1 cases to WHO as required by International Health Regulations. Finally, on December 28, 2009 the Ministry of Health in Indonesia, DEPKES, announced a total 20 human cases for 2009 with 19 deaths. See: http://www.flutrackers.com/for...

No information was ever released on any of these cases, and even now, two years later, WHO simply notes these case in its summary table as "Indonesia numbers indicate cumulative total of sporadic cases and deaths which occurred during 2009."  The translation in paragraph 2 below sheds light on two or perhaps three of these 20 cases from Indonesia in 2009. The translation indicates that there was a family cluster of H5N1 infections in early June (?) 2009 in West Jakarta where a mother, her child, and a nephew were infected. The mother died and the two children apparently survived.

Obviously, this cluster did not result in a human outbreak of H5N1 in Jakarta, but the lack of timely reporting of H5N1 cases by the Indonesian government is disturbing. Also, the fact that two individuals in this cluster survived, yet the official DEPKES report only identified one survivor in 2009, calls into question the official reporting policies for H5N1 cases by the government of Indonesia.

SAMARINDA: Department of Animal Husbandry in East Kalimantan added 16 officers to the participatory disease surveillance and response or rapid reaction personnel handling infectious animal disease outbreaks of avian influenza or bird flu in 2011.

Influenza Vaccines Without Needles

NIH Study Provides Evidence for Universal Vaccine

Immune response against the H5N1 bird flu vaccine greatly increased in healthy adults if they are first primed with DNA vaccines expressing the gene for a key protein of H5N1, researchers say.  Their report describes the results of two clinical studies conducted by researchers from the National Institute of Allergy and Infectious Diseases (NIAID), part of the National Institutes of Health.
Most of the study volunteers who received the DNA vaccine 24 weeks before receiving the booster vaccine made of whole, inactivated H5N1 virus produces high levels of antibodies considered to be protective against the globular head region of a protein called hemagglutinin (HA).  Traditional seasonal influenza vaccines are designed to get the antibodies to the HA head region, but it changes every year so the vaccine must be repeated annually to maintain immunity.  In some volunteers, prime-boost vaccine regimen also spur the production of broadly neutralizing antibodies that aims to stem the HA, a region of relatively constant in many strains of influenza virus.

"The results of this study demonstrate an important proof of concept, in that it is possible to obtain broadly neutralizing antibody to influenza in humans through vaccination," says NIAID Director Anthony S.  Fauci, MD "These findings mark the beginning but significant milestone on the path to universal influenza vaccine that provides protection against multiple viral strains."

Findings from Phase I clinical trials appearing in the online October 4 article in The Lancet Infectious Diseases. Gary J.  Nabel, MD, Ph.D., director of NIAID's Vaccine Research Center (VRC), and his colleagues developed a DNA vaccine of H5N1 influenza.  Other vaccines used in this study were made by Sanofi Pasteur, which is located in Swiftwater, Pa.  In 2010, VRC studies in mice, ferrets and monkeys showed that DNA prime-boost regimen of influenza vaccine can cause a broad neutralizing antibodies directed against the HA stem.  

"Now we see that it is possible to obtain stem-HA antibody directed at people too," says Dr. Nabel.  VRC researchers hope to apply this approach to research on vaccines against other strains of seasonal and pandemic influenza as well.

Since 2003, there were 564 confirmed cases of H5N1 human influenza infections and 330 related deaths worldwide, according to latest figures from the World Health Organization.  Developing an effective vaccine against influenza H5N1 has proven difficult because the vaccine contains a virus, inactivated whole often fail to produce high levels of protective antibodies in people.  VRC studies confirm that only volunteers who received two doses of H5N1 vaccine viruses are inactivated within 24 weeks apart produced only modest levels of antibody directed H5N1.
