Avian influenza - situation in Egypt - update

7 June 2012 - The Ministry of Health and Population of Egypt has notified WHO of a new case of human infection with avian influenza A(H5N1) virus.

The case is a four year-old female from Kfr -Elsheikh governorate. She developed symptoms on 25 April 2012, was admitted to a hospital on 26 April 2012 and received oseltamivir treatment upon admission. She was discharged from the hospital on 7 May 2012.

Investigations into the source of infection indicated that the case had exposure to backyard poultry. (continued)

Jakarta, Indonesia:  An 8-year-old Indonesian girl has died from the H5N1 bird flu virus, health officials said Thursday.  The girl, identified as K.K., fell ill June 18, six days after carrying home five recently butchered chickens from the Karawang Market in West Java, Indonesia's Antara news agency reported.  Tjandra Yoga Aditama, director general of disease control and environmental health at the Indonesian Health Ministry, said it's likely she contracted the disease at the market.

"She was in contact with live poultry," Tjandra said.  "She went to the market with her father and sibling and bought five live animals.  She chose to hold the chickens after they have been killed."

The girl first became ill during a trip to Singapore.  A doctor there diagnosed her with laryngitis, Tjandra said.  By June 24 she was admitted to a hospital in Jakarta with a high fever, persistent cough and nausea.  Doctors believed she had pneumonia, Antara reported.  K.K. tested positive for the H5N1 virus June 29.  

"Her condition got worse and on July 3, 2012 at 22.45 she died," Tjandra said.

6 July 2012 - The Ministry of Health of Indonesia has notified WHO of a new case of a human infection with avian influenza A(H5N1) virus.

The case is an 8 year-old female from the province of West Java. She developed fever on 18 June 2012 and then travelled on vacation the following day to Singapore, where she saw a private physician who diagnosed pharyngitis on 20 June. The case returned to Jakarta on 24 June and was still feeling unwell with coughing, decreased appetite and vomiting. Her family took her for treatment to the local hospital. Her condition deteriorated and she was transferred to intensive care, but died on 3 July. Infection with avian influenza A(H5N1) virus was confirmed by the National Institute of Health Research and Development (NIHRD), Ministry of Health. (continued)

SINGAPORE: The Health Ministry says it has been informed of a case of avian influenza A (H5N1) in an eight-year-old girl from Indonesia who had travelled to Singapore. In a statement, the ministry said the girl developed fever on June 18 in Indonesia. She was in Singapore from June 19-24 on vacation. She returned to Indonesia on June 24, and later died on July 3 this year.

The Indonesian health authorities have reported that the girl had exposure to poultry while in Indonesia, a few days before her travel to Singapore. They are monitoring her close contacts, including family members. All contacts have remained well.  As a precautionary measure, Singapore's Health Ministry has alerted hospitals to the case and commenced contact tracing in Singapore.  The girl stayed at a hotel in Singapore with her mother and other relatives from 19 to 24 June 2012 on a vacation.  She was brought to see a general practitioner for high fever on 22 June and was given treatment. Her relative who lives here and the general practitioner who treated the case are both well.

The WHO report on this girl (see above) states that she was symptomatic before leaving for Singapore. Therefore, she was infection in Indonesia, not while she was traveling outside the country.

The case is a 37 year old male from Yogyakarta province. He developed fever on 24 July 2012, was hospitalized on 27 July and died on 30 July.

Epidemiological investigation on the case found that the case had four pet caged birds in his home, which is about 50 metres from a poultry slaughter house and near a farm.
Infection with avian influenza A(H5N1) virus was confirmed by the National Institute of Health Research and Development (NIHRD), Ministry of Health and reported to WHO by the National IHR Focal Point.

Does this mean 97 are suspected of bird flu, rather than confirmed?

The first few translations were alarming because it stated they all had bird flu as if this were a confirmed fact, but this last article sounds as if what has happened is 250 birds died, and simultaneously 97 people in the area came down with ILI, so at this point bird flu is suspected but not confirmed...is this correct?

(It's still very worrisome, but I was ready to duck and cover if it were a confirmed cluster of almost 100)

To err on the side of caution, I would assume that these people are only showing symptoms.  I do wonder what tests have been done and if they have had time to get lab test results. Nevertheless 97 people with symptoms is still serious.

You really can't tell at this point.

We've seen many situations that seem dire but so far none of them have led to a breakout they couldn't control. There is just no way to tell at this point. We'll just have to watch the situation and hope they give us some accurate information soon. You can't count on that from Indonesia. The new Health Minister seems good but she just hasn't been there long enough to tell how she is going to handle release of information on flu burung cases.

Eight Year Old Dies of Suspected Bird Flu

Yogyakarta, Indonesia: The case of bird flu (H5N1) in the DIY in the past four months has left two people dead.  Both come from the Sleman district where one person tested positive for H5N1, and the other one is a suspect H5N1.  Head of Disease Control and Health Issues (P2MK) DIY Health Department Daryanto Chadorie revealed that one positive case of H5N1 occurred about four months ago in the area of Prambanan, Sleman.  The patient was a man about 39 years old who died in Bethesda hospital.

"The other one is still a suspect H5N1 case, a toddler.  Blood samples also were sent to health laboratories in Jakarta last week. Yet until now there has been no result," he said, Sunday (16/12)

According to him, because of the deaths of two people with bird flu, the case should have been declared an Extraordinary Incident (KLB).  However, the decision to declare the outbreak is contained in each county/city.

"Although there has been no declaration of bird flu outbreaks, the handling is done as an outbreak of bird flu in Prambanan.  When patients tested positive for bird flu, the epidemiological investigation (began) immediately," he said.

Meanwhile, the case of the death of a toddler allegedly because of AI was also expressed last week by H5N1 Team Prevention H5N1 Dr Dr. Dr. Sardjito Sumardi, SpPD (K).

"A week ago there was information that the infant suspected of bird flu would be referred to the department of Sleman Sardjito.  However not yet (before he) reached the department of Dr. Sardjito, the patient died. Due to the death not (occurring in) Dr Dr Sardjito, then we did not do a sample," he added.

Jakarta, Indonesia:  A 4-year-old Indonesian boy has died from bird flu, bringing the death toll to 160 in the country hardest-hit by the deadly virus, a health official said on Tuesday

The boy died on 6 December in Tangerang city, just west of Jakarta, the capital, said health ministry official Rita Kusriastuti.  He developed symptoms of a cold and fever on 30 November and was treated at a public health center before being hospitalized the same day he died.  Kusriastuti said the boy, from the West Java district of Bogor, was believed to have been infected with the H5N1 virus after having direct contact with dead fowl around his house.

Last week, Kusriastuti said a form of the H5N1 virus not previously detected in Indonesia had killed hundreds of thousands of ducks on the main island of Java.

In a sketchy monthly update, the World Health Organization (WHO) yesterday confirmed an H5N1 avian flu case in a 2-year-old Egyptian girl earlier this month and an H5N1 death in a 4-year-old Indonesian boy that local media had reported this week. The few details that the WHO provides about the girl, from Demietta governorate, are confusing. The agency lists her date of illness onset as Dec 3, but it also says she was hospitalized on Dec 1. It says she received oseltamivir (Tamiflu) on Dec 4 but doesn't specify whether she has recovered. Her case is Egypt's 11th this year, 5 of which have been fatal, according to WHO data. The Indonesian boy, from West Java, became ill Nov 30, was hospitalized Dec 5, and died Dec 6, the agency said. Indonesia has now confirmed nine H5N1 cases this year, all fatal. The two countries lead the world in 2012 H5N1 cases, with Vietnam, at four cases, reporting the third-highest number. Since 2003, the WHO has confirmed 610 cases, 360 of which (59%) have been fatal. The WHO's previous update was Nov 5. The agency recently said it would report global cases monthly.

The Central Java Health Agency says it has received a report of a suspected case of bird flu in the province.
The 54-year-old man, a resident of the Sambiroto subdistrict, was currently under treatment at St. Elisabeth Hospital in Semarang, officials said.
"The man has been suffering from a cough, a cold and a high fever, with temperature above 38 degrees Celsius," Rorry Hartono, the agency's disease control and environmental health division chief, said on Wednesday.
More than 62,000 ducks have thought to have died from the H5N1 virus in 21 regencies and cities in the province as of Tuesday, according to officials.

Central Java Animal Husbandry Agency head Whitono said that the virus, which apparently mutated since it was brought to Indonesia via migrating birds, has spread quickly since the first case was reported in the province in September.

"This case is quite new because bird flu commonly infects chickens," he said.
The Central Java Agriculture Agency has distributed 9,300 liters of disinfectant, calling on farmers to report any ducks who have recently died to the agency.

Semarang, Indonesia:  Suspected (of contracting the) bird flu virus, a man with the initials of Semarang District Court is undergoing treatment at the Bubble Vincent Elizabeth Hospital in Semarang, Central Java.  The male patient came with symptoms and complaints of cough, cold and hot with high temperatures.   He was hospitalized Monday (17/12) until now.

"The patient has a farm in the area Bergas, Semarang regency, with three or four dead chickens. Fearing his condition and his shortness of breath, the man reported," said Public Relations Elizabeth Hospital Semarang Probowati Tjondronegoro reporters in his office, Thursday (20/12).

Personnel at RS Elizabeth finally reported to the City Health Office (DKK) Semarang that directly fielded a team to inspect the condition of the Constitutional Court.  Based on the results of the photo such as piston Elizabeth Hospital and the other shows the patient's condition is
good.  While the results of the examination of the DKK team are still undergoing laboratory tests, the results will be known on Saturday (22/12) tomorrow.

"If there is a (positive result), then we'll move (refer) to RSUP Kariadi because there is a room for tackling patients with suspected bird flu," he said.

Chief Medical Officer (PHO) of Central Java Province Anung Sugihantono describes that this is the first time in the year 2012 that a confirmed case of suspect bird flu that hit the man with the initials MK in Elizabeth Hospital.

"In epidemiology, the Court considers this suspect because of a history of contact with dead poultry.  Governance (patients) as well as bird flu, its handling is split," he said on Thursday (20/12).

Examination of the health department has been done.  But the results of the examination of suspected bird flu or H5N1 could only come out on Saturday (22/12) future.

"We have not yet dared to make sure the relevant (MK) contracted the bird flu virus and is a suspect. Let's wait for the result on Saturday (22/12) tomorrow," he concluded.

Central Java, Indonesia:  For the second time, a resident in Central Java is suspected of bird flu virus and isolated in hospital.  The second victim this time is an older new child (ABG), 15 years old.  The boy, with initials MAK, had to be treated in hospital isolation room at Doctor Soewondo Kendal Regency, Central Java.

Doctors Hospital Soewondo Kendal PR, Endang Setyorini confirmed to merdeka.com on Saturday (22/12) that the patient came to the hospital on Friday (21/12).  The patient complained of head dizziness, cough, runny nose and body high heat.

"His complaint was initially really like the common flu.  However, the fact that the complaint appears to say there were a lot of birds in the surrounding environment that died suddenly, the patient was immediately placed in isolation.  We are concerned if this is positive bird flu," he said.

According to Muarifin (15), who is a relative of MAK, the patient comes from Pare district of Kediri Regency, East Java.  Since Monday (17/12) he and MAK were at the home of relatives at Hamlet Dodokan RT 2 RW 5 Navan Rural District Patebon Kendal to attend a niece's wedding.

"But on Friday morning MAK suddenly complained of pain.  It is a fact we had never been in direct contact with poultry but often by chance we see a duck coop adjacent.  Before complaining hot, so amazing.   On Thursday we even had time to play PS (playstation) together. MAK was brought to the health center, but was subsequently referred to the hospital," said Muarifin.

Deputy Director of Field Services Kendal Soewondo Doctors Hospital Saeikhu said that in addition to the space, a special diet for the patient is also considered so his condition does not drop.

"Currently, blood and saliva samples of the patient are being conducted for laboratory tests in hospitals Dr. Kariadi Semarang.  Samples have been sent.  To determine if it is positive or not, we wait for the results," said Saekhu.

Shaeku added that MAK is the first patient in Soewondo Hospital in 2012 suspected of bird flu. But to anticipate the surge in bird flu suspect patients, they have set up some other isolation.
Previously, as reported to merdeka.com, Kendal DVO poultry in six states had tested positive for bird flu.  Additionally there were Patebon, the District Weleri, Rowosari, Cepiring, South Kaliwungu and Brangsong.

With suspect MAK, the Kendal resident is isolated due to the AI virus in Central Java had two patients undergoing treatment in the isolation room at the two hospitals. The first alleged AI suspect victim was MK, a Bergas citizen, Semarang regency who is a poultry farmer.  MK is undergoing treatment in the isolation room Semarang Elizabeth Hospital since Thursday (20/12) as a suspect patient who complains of AI-like virus.

Cilegon, Indonesia:  The public is asked to increase their vigilance against bird flu.  This is because the disease caused by avian influenza (AI) is thought to have begun infecting people.
Yesterday, Purnama Sari Indah (22), a resident of Environment Ciberko, RT / RW 02/02, Village Kalitimbang, District Ciberer, was declared a bird flu suspect.  Since Wednesday (2/1), Purnama experienced fever, dizziness, and shortness of breath.  He suffered this after discarding some of his birds and poultry that died suddenly.  

"It's been a few days my chickens, ducks, and pigeons died suddenly.  Originally they had seizures with blue eyes, and then died.  Fearing the end I threw the carcasses into the river near the house.  I threw the chicken carcasses last Wednesday (2/1) afternoon," Purnama told the clerk at the Department of Agriculture and Marine (Disperla) Cilegon who visited yesterday.

After hearing Purnama's explanation, the officer immediately checked his blood birds.  The Disperla officer who arrived related reports of the birds that died suddenly in Kalitimbang.  

"I did have a report to Disperla that my birds died suddenly.  I suspect that the chickens and ducks had bird flu, even now it seems I am also affected by bird flu," said Purnama.

He admitted that he had to check his health to the health center Ciberer.  Personnel referred him to the hospital clinic Cilegon, on Village Panggungrawi, District Jombang.  In the Cilegon hospital Purnama had a blood test.  Unfortunately, he claimed not to know the results of the test.  

"After the blood was taken, I went home.  The problem is, after the blood tests at night, I want to break a strong baseball," he complained.

Hospital Director Dr. Zainoel Arifin Cilegon contacted last night said the blood tests performed in the Ministry of Health Laboratory (MoH).  

"Directly we send this to the Ministry of Health, because we do not have the tools for the observation of bird flu," he said.

Meanwhile, Head of Control and Environmental Health (P2PL) at the Department of Health (PHO) Cilegon Retno Windarwati admitted monitoring the progress of the illness Purnama. Therefore, he was not sure when Purnama (contracted?)  H5N1 virus (bird flu).  

"The processed blood tests are the property of Purnama, until now we have not received the results.  Because of the illness has not been confirmed that Purnama has bird flu," he said.  "We are also awaiting the results of the examination by Disperla Cilegon related the the birds that died suddenly.  If the birds died from bird flu, we will take precautions so it will not attack humans," added Retno.

In the meantime, Head of Animal Health (Keswan) and Public Health Feteriner (Kesmafet) on Disperla Cilegon Dr. Dina Safitri Animals recorded more than 20 birds and chickens that died suddenly in the Citangkil and Ciberer.  They are now investigating the case.  

"We've taken some blood samples.  Now we must test to find out if indeed this is a bird flu attack," he said.

Dina said the case is different from previous years.  This time the alleged attack of bird flu is more contagious to waterfowl, namely ducks.  

"Usually it is attacking chickens, but now the water fowl get this attack," he said.

He said Cilegon is not an endemic area of bird flu.  Moreover, during 2012 it accounted for only ten cases of alleged reports of bird flu.  Nevertheless, he asked people to keep improving alertness.  Therefore, this rainy season increases the potential for bird flu.  

"The attacks of bird flu are common in the rainy season.  Therefore, I hope people who raise poultry are more diligent in cleaning cages, washing well with coir.  Birds must always be in the cage, so that if there is a sudden death, it does not spread to other areas," he said.

PHNOM PENH, CAMBODIA Bird flu has infected three humans in Cambodia, killing two, health officials said Friday.

The cases that have occurred in the first three weeks of this year amount to as many cases the country reported throughout all of 2012.

The cases are among the first reported in 2013 for the virulent virus, known as H5N1, which the World Health Organization says has killed 360 other people worldwide since surfacing in 2003.

WHO and Cambodia's health ministry announced that a 15-year-old girl in a village in southeastern Takeo province and a 35-year-old man in central Kampong Speu province died after being hospitalized with bird flu. An 8-month-old boy in the capital, Phnom Penh, was treated and survived.

Cambodia reported three cases last year, all of them fatal. Since 2005, it has recorded 21 cases, 19 of them fatal.

The first case, an 8-month old infant boy from Chrey Korng Village, Sangkat Chorm Chao, Khan Por Sen Chey, Phnom Penh has been diagnosed with H5N1 influenza. He developed symptoms on 08th January 2013 then was brought to the National Paediatric Hospital for consultation on 9th January with fever, cough, runny nose, and vomiting. The boy was registered in the Influenza-Like Illness Sentinel Surveillance and the samples were sent to the National Institute of Public Health's laboratory on 11th January. The result was confirmed by Institut Pasteur du Cambodge on 22nd January 2013. The infant has recovered and had history of coming into contact with poultry prior to becoming sick.

The second case, a 15-year-old female from Snao village, Snao commune, Prey Kabass district, Takeo Province, has been diagnosed with H5N1 influenza on 22nd January 2013. She became sick on 11th January suffering with fever and cough. She was initially treated by local private practitioners. Her condition worsened and she was admitted to Kantha Bopha Hospital on 17th January with fever and shortness of breath. Unfortunately, despite intensive medical care, she died on 21st January. There is evidence of recent deaths among poultry in the village and the patient prepared sick chicken for food prior to becoming sick.

In the third case, a 35-year-old man from Trapeang Sla village, Preah Nipean commune, Kong Pisey district, Kampong Speu province has been diagnosed with H5N1 influenza on 23rd January 2013 by Institut Pasteur du Cambodge. He became sick on 13th January, 2013 suffering with fever and cough. He was initially treated by local private practitioners. His condition worsened and he was admitted to the Khmer-Soviet Friendship Hospital in Phnom Penh with fever and dyspnea on 21st January. Samples were taken the same day and sent to the National Institute of Public Health's laboratory. Despite intensive medical care, the patient died soon after the samples were taken. There is evidence of recent deaths among poultry in the village and the patient prepared sick chicken for food prior to becoming sick. The man is the twenty-fourth person in Cambodia to become infected with H5N1 virus, and the third person this year and the twenty-first person to die from complications of the disease.

Of all the twenty four cases [to date], 15 were children under 14, and fifteen of the twenty four confirmed cases occurred in females.

(Note: These human cases most likely are caused by the newer clade 2.3.2 strain out of China. (JMO) That clade has spread in the poultry over a large portion of Vietnam during the past year. There were 4 confirmed human cases of H5N1 in Vietnam in 2012 and 2 of them died. This same clade has been found recently in the poultry in Indonesia. Confirmed human cases of H5N1 in Indonesia totaled 9 with 9 deaths in 2012. Cambodia had 3 confirmed human cases in 2012 with 3 deaths and now 3 new cases with 2 deaths in 2013. IMO, the clade 2.3.2 is the scariest strain to evolve so far.)

Two Cambodians die from bird flu: WHO

Two Cambodians have died from bird flu (Snip), the World Health Organization said Friday.

Tests on the victims, a 15-year-old girl and a 35-year-old man who died earlier this week, confirmed they had contracted the H5N1 strain of avian influenza (Snip).

An eight-month-old boy admitted to hospital in Phnom Penh on January 9 was also infected with H5N1 but later recovered (Snip).

There was evidence of infections among poultry in the villages of the two who died and the pair "prepared sick chicken for food prior to becoming sick", (Snip).

Cambodia has second 2013 bird-flu death

PHNOM PENH, Cambodia, Jan. 28 (UPI) -- (Snip) a teenage girl who suffered from bird flu died Monday, the second death from the H5N1 virus this year.

There have been four reported cases of the avian disease and that is probably the "tip of the iceberg," said Dr. Philippe Buchy, head of the virology unit at the Institut Pasteur du Cambodge that tested the recent cases.

The girl, 15, was from the village of Snao in Takeo province.

On Saturday, more than 4,000 chickens and ducks were killed and burned outside her village, district Gov. Ith Sa said. The village was sprayed and medicine was distributed to eradicate the virus, the governor said.

The other person killed by the disease since the beginning of the year was a 35-year-old man (Snip).

Bird flu kills two more children in Cambodia

The Cambodian office of the U.N.'s World Health Organization said a 17-month-old girl from central Kampong Speu province and a 9-year-old girl from southern Kampot province died Monday after being hospitalized.

Last week, Cambodia reported three human cases of bird flu, two of them fatal. For of all 2012, the country reported a total of three cases, all fatal.

PHNOM PENH, 29 January 2013 - The Ministry of Health (MoH) of the Kingdom of Cambodia wishes to advise members of the public that two new more cases of avian influenza has been confirmed positive for the H5N1 virus.

The fourth case is a 17-month-old girl from Prey Nheat village, Prey Nheat commune, Kong Pisey district in Kampong Speu province has been diagnosed with H5N1 influenza on 26th January 2013 by Institut Pasteur du Cambodge. She developed symptoms on 13th January 2013 with fever, cough, runny nose, and vomiting. She was initially treated by local private practitioners. Her condition worsened and she was admitted to Kantha Bopha Hospital on 17th January with fever, cough, somnolence, and dyspnoea. Unfortunately, despite intensive medical care, she died on 28th January. There is evidence of recent deaths among poultry in the village and the girl had history of coming into contact with poultry prior to becoming sick.

In the fifth case, a 9-year-old girl from Thmei village, Thmei commune, Toeuk Chhou district, Kampot province has been diagnosed with H5N1 influenza on 28th January 2013 by Institut Pasteur du Cambodge. She became sick on 19th January, 2013 suffering with fever and cough. She was initially treated by local private practitioners. Her condition worsened and she was admitted to Kantha Bopha Hospital with fever cough, somnolence and dyspnoea on 27th January. Despite intensive medical care, the patient died on 28th January. There is evidence of recent deaths among poultry in the village. This girl is the twenty-six person in Cambodia to become infected with H5N1 virus, and the fifth person this year and the twenty-three person to die from complications of the disease. Of all the twenty six cases, 17 were children under 14, and seventeen of the twenty six confirmed cases occurred in females. (snip)

The Ministry of Health's Rapid Response Teams (RRT) have gone to the hospitals and the field to identify the patient's close contacts, any epidemiological linkage among the three cases and initiate preventive treatment as required. (continued)

New Bird Flu Case Raises Fears in Cambodia

PHNOM PENH -  (January 28, 2013 ) Two Cambodians have already died from bird flu in 2013, making a worrying start to the year. Now a two-year-old Cambodian girl is in a serious condition in Phnom Penh after being hospitalized with the H5N1 virus (Snip).

Sonny Inbaraj Krishnan, the communications officer for the World Health Organization in Phnom Penh, says the development has health professionals concerned. "This is the fourth case this month of human influenza H5N1," Krishnan said. "Last year we had three cases, so within one month in the new year we've got four cases, and we're quite concerned about that."

The latest victims here were a 15-year-old girl, who died a week ago, and a 35-year-old man, who died last Wednesday. A baby who fell ill earlier in the month has recovered. Over the weekend officials culled and burned more than 4,000 chickens and ducks in the village that was home to the 15-year-old victim.

However, other diseases such as dengue and malaria kill many more people than H5N1, so Krishnan was asked what is the concern with avian flu? "Well our specific worry is that this H5N1 virus could undergo what we call a recombination and then re-assortment with another influenza virus," he explained, "and that could give rise to a new virus that is transmittable between humans - so that's our main concern."

Cambodia is a predominantly agricultural nation, and every village has its chickens and ducks. Health ministry staff are monitoring those who came into contact with the patients who were infected, and teams from the agriculture ministry are testing poultry in the affected villages and destroying sick birds.  

On Friday Health Minister Mam Bunheng called on parents to ensure their children wash their hands regularly, and stay away from sick and dead poultry. He also advised that children who develop breathing difficulties should be taken directly to the nearest health clinic.

Krishnan says TV and radio are being used to spread that message. "So from this week onwards we're going to increase the number of radio and TV spots - telling them how to protect themselves and their families from avian influenza," he explained. "Especially to watch out for children playing with chickens - and also a very important message is to wash your hands."

Cambodia reported its first cases of H5N1 in 2005 when four people died. To date the worst year was 2011 when eight people were infected. All eight died.

The country's weak health sector is a hindrance and likely goes some way to explaining why Cambodia's avian flu fatality rate of nearly 90 percent - 21 dead from a total of 24 infected - is so much higher than the global average of around 60 percent.

Cambodian bird flu deaths prompt awareness drive

PHNOM PENH, 28 January 2013 (IRIN) - Health authorities in Cambodia will bolster public awareness campaigns on H5N1 avian influenza after four people became infected in January, resulting in two fatalities. "Ongoing public awareness campaigns need to be reinforced through TV and radio," Sok Touch, director of Cambodia's Communicable Disease Control Department (CDC), told IRIN on 28 January, calling on people to be vigilant. "We're planning on doing this immediately as there is no room for complacency."

The four cases of H5N1 avian influenza, commonly known as bird flu, are the first confirmed in Cambodia this year. There were three recorded cases (all fatal) in 2012.

Since 2005, 24 people have been infected resulting in 21 deaths, according to WHO, with over half of the infections in children under 14.

According to a joint statement from the Ministry of Health and World Health Organization (WHO) on 25 January, an eight-month-old boy from the capital Phnom Penh recovered after being infected with bird flu, while a 15-year-old girl from southwestern Takeo Province and a 35-year-old man from southwestern Kampong Speu Province died after contracting the virus.

The CDC said the boy had contact with chickens at a market, but the girl from Takeo and the man from Kampong Speu both fell ill after cooking dead chickens gathered from their villages. A fourth case, also in Kampong Speu, was confirmed by the Ministry of Health on 27 January, when a 17-month-old girl tested positive for H5N1.

"We are working closely with the Ministry of Health to enhance surveillance of H5N1," said Sonny Krishnan, communications officer with WHO in Phnom Penh, adding that WHO did not know yet if there was a link between the cases of the girl and 35-year-old man. Padang, Indonesia:  Five toddlers in the city of Padang are allegedly infected with the bird flu virus and hospitalized in Muhammad Jamil.  The five children whoare allegedly infected with bird flu in the city of Padang, allegedly had contact with dead poultry.  They were treated intensively in isolation with the suspected disease in hospital M Jamil.  Some of them had been treated for three days.

Until now, their condition is not stable and they are still experiencing shortness of breath and high fever. To make sure whether they are positively infected by the H5N1 virus, blood samples have been sent to a hospital in Jakarta.  Based on information from the patient's parents, their sons had direct contact with chickens that died suddenly in the home environment.  Anticipating the spread of the H5N1 virus, the Padang Animal Husbandry Department has been spraying and checking poultry in the traditional markets.  Since its inception in 2011, they have found 10 cases of birds that died suddenly in the city of Padang.  Dozens of dead birds were positively infected with the H5N1 virus.  Traders are advised to keep them in their chicken coops.

PHNOM PENH: A five-year-old Cambodian girl has died from bird flu, bringing the country's toll from the deadly virus to five so far this year, the World Health Organisation (WHO) said on Friday.

The girl, from the southern province of Takeo, died on Thursday in hospital and tests confirmed that she had contracted the H5N1 strain of avian influenza (Snip).

She had come into contact with poultry prior to becoming sick, the statement said.

Bird flu deaths reach five in Cambodia

The fifth fatality from the H5N1 virus was a 5-year-old from Takeo province, who died in hospital on Thursday, said Sonny Inbaraj Krishnan, a spokesman for the World Health Organization (WHO).

A sixth victim recovered in January, he said, adding that no human cases of bird flu have been reported outside Cambodia this year.

A team from the Ministry of Health had travelled to the girl's village to test residents and advise them on how to avoid infection, he said.

"They will take swabs and draw blood and test it for the virus soas to rule out human-to-human transmission." All human cases of H5N1 have been infected by contact with poultry, but health professionals fear the virus could combine with another or mutate and gain the ability to pass between people.

PHNOM PENH, 8 February 2013 - The Ministry of Health (MoH) of the Kingdom of Cambodia wishes to advise members of the public that one new more case of avian influenza has been confirmed positive for the H5N1 virus.

The sixth case is a a 5-year-old girl from Angk Krasang village, Prey Lvea commune, Prey Kabass district in Takeo province has been diagnosed with H5N1 influenza on 7th February 2013 by Institut Pasteur du Cambodge. She developed symptoms on 25th January 2013 with fever, cough, and vomiting. She was initially treated by local private practitioners. Her condition worsened and she was admitted to Kantha Bopha Hospital on 31st February with fever, cough, and dyspnoea. Unfortunately, despite intensive medical care, she died on 7th February. There is evidence of recent deaths among poultry in the village and the girl had history of coming into contact with poultry prior to becoming sick. The girl is the twenty-seven person in Cambodia to become infected with H5N1 virus, and the sixth person this year and the twenty-four person to die from complications of the disease. Of all the twenty seven cases, 18 were children under 14, and eighteen of the twenty seven confirmed cases occurred in females. (continued)

Djamil Padang.  The senior, 73 years old with the initial D, is an alleged bird flu virus (H5N1) suspect.  Giving Information and Documentation Officer (PPID) Dr M.  Djamil Padang, Gustafianov, confirmed they are nursing a bird flu suspect patient, who is now in the isolation space.  

"From the symptoms he suffered suspected bird flu, lest other people are now infected with the same patient in isolation.  The symptomsare  similar to bird flu," he told Okezone, Saturday (02/09/2012).

He continued that today they are giving intensive care for the patient while waiting for laboratory results.  "We are still awaiting the results of the examination in the laboratory, whether these patients truly affected by bird flu or not," he said.

Gustafianov continued that the patient was admitted to the hospital on Friday, February 8 at approximately 12:00 noon EDT.  From the description of the family, he continued, the patient had three days of high fever, coughing, and difficulty breathing.  "Initially, the patient was taken to the hospital Padangpariaman, but the hospital referred the patient here," he said.

While based on information from the family, the patient was not in contact with farm animals.  Only, hundreds of ducks around at their home died suddenly on Wednesday, February 6 last.  Allegedly, the ducks were affected by bird flu.

BEIJING (Reuters) - Two people in southwest China have tested positive for the H5N1 bird flu virus and are critically ill, state news agency Xinhua said on Sunday, citing the Chinese health ministry.

A 21-year-old woman and 31-year-old man in Guiyang had tested positive for the highly pathogenic H5N1 virus on Sunday after developing symptoms on February 2 and February 3 respectively, Xinhua cited health authorities as saying.

"They are in critical condition and medical workers are carrying out emergency treatment," the health ministry was quoted as saying. "No epidemiological connections have been found between the two cases."

Millions of Chinese are currently travelling in crowded buses and trains across the country to celebrate the Lunar New Year, raising the risk of the spread of infectious diseases.

People who had close contact with the patients are under medical observation but none have fallen ill, the news agency said.

The pair did not have contact with fowls before they fell ill, Xinhua said. In most cases, victims had direct contact with infected birds, mostly chickens. The virus is especially abundant in the faeces and respiratory secretions of affected birds.

One H5N1 patient dies in SW China

GUIYANG, Feb. 13 (Xinhua) -- One of two people confirmed by the Ministry of Health to have contracted avian influenza H5N1 died in a hospital in southwest China's city of Guiyang Wednesday morning.

The patient, a 21-year-old woman, died of multiple organ failure at the Guizhou Provincial People's Hospital at 9:41 a.m. Wednesday.

The woman and the other patient, a 31-year-old man, are both Guiyang residents. They developed symptoms on Feb. 2 and 3, respectively. Both of them tested positive for the H5N1 virus on Sunday (Snip).

Epidemic control experts have not found any indication that the patients had close contact with birds, nor have they found any epidemiological connection between the two cases.

21-year-old woman dies of bird flu in Guiyang, China

A woman in southwestern China died of bird flu on Wednesday, state media said (Snip). The 21-year-old patient from Guiyang passed away 13 days after showing symptoms, Xinhua news agency said, citing the health ministry.

Another city resident, a 31-year-old man who developed signs of the virus around the same time, was reported over the weekend to be in critical condition.

The girl is the seventh person this year and twenty-eighth person to become infected with H5N1 virus, and the twenty-fifth person to die from complications of the disease in Cambodia. Cambodian health officials have reported an unusual rise in the number of bird flu cases this year following a young girl's death. It brings the country's death toll from the virus to six this year alone.

Mutation fears

"For this rise in case, we're working closely to rule out that we don't have any human-to-human transmission," he said.

Bangli, Indonesia:  MD (2.5) of Hamlet Sidawa Tamanbali village, Bangli was referred from hospital to Sanglah Hospital.  MD was referred to Bangli hospital on Wednesday (13/2) and then, with symptoms of fever, breathlessness and coughing since three days earlier.  After the lab examination in hospital Bangli and a history of poultry that died suddenly at the patient's home, hospital Bangli took shortcuts to refer him to the Sanglah Hospital.

Bangli Hospital CEO dr. Wayan Sudiana, on Thursday (14/2) yesterday admitted referring MD, a child who is a bird flu suspect to Sanglah Hospital, Wednesday (13/2).  The patient was referred to hospital Bangli by his family, on Tuesday (12/2) at 19:30 pm.  Diagnosis and the history that is taught to the medical team in charge of the lab tests were finally done.  In lab tests they conducted, it leads to the patient being a bird flu suspect.  Given the limitations of the equipment owned by hospital Bangli in detecting disease, they decided to refer the toddler to Sanglah Hospital Wednesday at 9:00 pm.

"We do not want to risk something happening.  Neither the safety of the toddler or the officers themselves.  Due to the detection of the bird flu virus, this has caused the disease to need more comprehensive treatment,'' he said.

Later after being referred to Sanglah Hospital, he will be tested in the form of test swab (Biomol), including checks by Research and Development Lab in Jakarta.  Meanwhile, the Bangli District Head of Livestock and Fisheries Ir. AA Ngurah Samba admits there are some villages in Bangli that have bird flu based on the positive results of rapid tests.  They have deployed officers to conduct spraying or biosecurity and control stock traffic in and out of the Bangli entrance. As well as regular spraying disinfectant in animal markets Bangli.

Padang, Indonesia:  A bird flu suspect patient was sent back to his village Kanagarian Padangpariaman Sanur district, yesterday (14/2).   The laboratory results stated he was negative for the H5NI virus negative.  Official giver of information and documentation (PPID) Dr Dr M Djamil Padang Gustafianof said the medical team that handled the patient named Darali, a resident of kenagarian Padangpariaman Sanur district left the treatment room yesterday afternoon.  This was done because the patient's condition was initially very weak and is now fully recovered.

According to the information summarized in Padang Express Dr M Djamil Padang, the cost of the treatment of patients is dealt with entirely by the State.  However, when questioning the Chief Medical Officer of West Sumatra, Rosnini Savitri, he said that for patients who tested negative for bird flu, the costs are not borne by the West Sumatra Health Office.  

"The treatment is borne by the patient's family and is not covered by the city of West Sumatra.  Since the results are negative, meaning this disease is a common disease.  For the cost of treatment, patients can use a card that has, for example Jamkesmas or Askes," he explained.

PR Elisabeth Hospital in Semarang, Probowati Condronegoro, said the patient went to the hospital since December 17 last year. Before the illness, the patient is known to raise poultry at home. Although he is still suspected to have symptoms similar to bird flu, the patient has been treated in accordance with governance bird flu.  Treatment is given according to the standard as antiflu, intact and oxygen delivery in the intensive care.

Feb 18, 2013 (CIDRAP News) - A 36-year-old Egyptian woman from Beheira governorate recently died from an H5N1 avian influenza infection, pushing the world's number of infections reported so far this year to 10, according to an update on the disease from the World Health Organization (WHO).

The woman started having symptoms on Jan 16 and was hospitalized and started on oseltamivir (Tamiflu) treatment on Jan 20. She died on Jan 26. An investigation into the source of her illness found that she had been exposed to sick poultry before she got sick. Beheira governorate is in northern Egypt's Nile Delta. (continued)

Cambodia reports 7th death of bird flu this year

Cambodia reported on Thursday that another 2-year-old boy died of Avian Influenza H5N1, bringing the death toll to seven and the number of cases to eight in 2013.

The toddler lived in Angkor Chey district of Southwestern Kampot province. He died of the virus in the early hours of Tuesday morning at the Kantha Bopha Hospital in Phnom Penh, 18 hours after he was admitted (Snip).

Last week, a 3-year-old girl from the same district also died of the virus at the hospital.

PHNOM PENH, 21 February 2013 - The Ministry of Health (MoH) of the Kingdom of Cambodia wishes to advise members of the public that one more new human case of avian influenza has been confirmed positive for the H5N1 virus.

The eighth case, a 20-month-old boy from Ang Romduol village, Angkor Chey commune, Angkor Chey district in Kampot province, was found positive for influenza H5N1 on 19th February 2013 by Institut Pasteur du Cambodge. He developed fever and runny nose on 6th February 2013 and was initially treated by local private practitioners. His condition worsened and he was admitted to Kantha Bopha Hospital on 18th February 2013 with fever, cough and dyspnea and died on 19th February 2013. There is evidence of recent deaths among poultry in the village.

The boy is the eighth person this year and twenty-ninth person to become infected with H5N1 virus, and the twenty-sixth person to die from complications of the disease in Cambodia. Of the twenty-nine confirmed cases, 20 were children under 14, and nineteen of the twenty-nine were female.

Second bird flu death in China

23 February 2013 - The deadly H5N1 strain of bird flu has killed a 31-year-old man, the second death reported in south-west China.

The Guizhou provincial health department said on its website that the man died of multiple organ failure yesterday at a hospital in the city of Guiyang.

The man was the second of two people to have died in Guiyang after contracting bird flu early this month. The first fatality was a 21-year-old woman.

The official Xinhua News Agency says both patients had close contacts with birds.

Bird flu kills 2nd victim in China

Beijing - A man in southwestern China has died of bird flu, health authorities said on Saturday, becoming the second fatality from the H5N1 virus this year.
The 31-year-old died in hospital in the city of Guiyang on Friday, the Guizhou province health department said in a statement, adding that no other human cases of avian flu had been reported in the province.

Another city resident, a 21-year-old woman, died from the virus earlier this month.

China is considered one of the nations most at risk from bird flu epidemics because it has the world's biggest poultry population and many chickens in rural areas are kept close to humans.

China has in the past been accused of covering up the extent of bird flu outbreaks, exacerbating fears when new cases are reported.

Cambodia reports 9th bird flu case

PHNOM PENH: Cambodia reported on Tuesday that a 35-year-old man died of Avian Influenza H5N1, bringing the death toll to eight and the number of cases to nine in 2013 (Snip). The man, identified as Thoeun Doeun, lived in Trung Trang district of Eastern Kampong Cham province.    

"He died of the bird flu virus last night after a four-day medical treatment at the Calmete Hospital in Phnom Penh," Suon Sokhy, younger sister of the victim, told China's Xinhua news agency over telephone. She said that her brother became sick after he took a dead duck to cook and ate. "Doctor told me that he died of bird flu," she said.    

Sonny Krishnan, communications officer with the World Health Organization (WHO) in Cambodia, confirmed the death on Tuesday.    

Meanwhile, he appealed to people to be high vigilant over the virus, saying that home slaughtering and preparation of sick or dead poultry for food is hazardous.

He added that parents and guardians should keep their children away from poultry -- do not allow them to touch feathers and do not keep poultry in their house.

"I'd like to urge people not to eat ill or dead poultry," (Snip). Poultry must be cooked well. Properly cooked poultry meat is therefore safe to consume."

9th Cambodian case details

The ninth case, a 35-year-old man from Kbal Ou village, Me Sar Chrey commune, Stueng Trang district in Kampong Cham province, was confirmed positive for influenza H5N1 on 23rd February 2013 by Institut Pasteur du Cambodge. He developed fever on 8th February 2013 and his condition worsened on 10th February 2013 with fever, frequent cough, and dyspnea. Local private practitioners initially treated him but his condition further deteriorated. On 13th February he was admitted to the Kampong Cham Hospital with fever, severe cough and dyspnea and was immediately treated with Tamiflu. He developed pneumonia on 21st February and was transferred to Calmette Hospital in Phnom Penh. Unfortunately, despite intensive medical care he died on 25th February. There is evidence of recent deaths among poultry in the village and the man had history of coming into contact with sick poultry prior to becoming sick. The man is the ninth person this year and the thirtieth person to become infected with the H5N1 virus, and the twenty-seventh person to die from complications of the disease in Cambodia. Of the 30 confirmed cases, 20 were children under 14, and 19 of the 30 were female.

Sukoharjo Resident Tested Positive for Bird Flu

Central Java, Indonesia:  A resident of District Mojolaban, Sukoharjo, Central Java initials S has tested positive for bird flu virus (avian influenza / AI).  Currently, the 70-year-old grandfather,  is receiving special treatment in the isolation room Moewardi Hospital Dr. Solo.  Public Relations Moewardi Hospital Dr., Dr. Eliza, said S has been in intensive care since three days ago.  S previously never had a history of contact with dead birds infected with AI.

"Earlier, he did make contact with dozens of dead chickens that were exposed to AI. Thereafter, S is in pain and is referred to here to get special treatment," Eliza told reporters on Friday (10/5).

Based on the results of a team of doctors, said Eliza, blood leukocytes for S are far below normal.  However, from the examination, fthey ound no pneumonia.  As for the results of laboratory tests and a throat swab, results have not been received.

"During our three-day hospitalization, development conditions are good. Nevertheless we will continue to implement the status of the patient as a bird flu suspect until laboratory results and swab are received," he said.

For the next status, said Eliza, the Hospital is still awaiting the results of laboratory tests and swabs.  Regarding the cost of care for a patient who is declared suspect, Eliza disclosed that all medical expenses will be borne by the state.

Two residents from Mojolaban, Sukoharjo were also referred to Hospital Dr. Moewardi Solo by Sukoharjo District Health Office, since Wednesday (8/5).   They referenced that the two residents suffered flu symptoms after the deaths of a number of chickens in the Village area Wirun, Mojolaban in the last 3 days.

Chief Medical Officer Sukoharjo, Guntur Subiyantoro dr said two people were admitted to hospital in Solo Moewardi, one of them named Mardi (73), a resident of Ngambak Kalang, Wirun Village, Mojolaban, as well as a resident for whom they are still confirming his name and address.

"Initially we only have the name Mardi who was referred to RS Moewardi.  However no report again if there is one citizen who suffers the same symptoms, and was also referred to the hospital Moewardi.  Chickens that died suddenly were positive for bird flu," said Guntur on Thursday ( 9/5).

Earlier, dozens of chickens in Ngambak Kalang, Wirun, Mojolaban died suddenly in the past week.  Dozens of dead chickens were positive bird flu (Avian Influenza / AI).  Head of Agriculture Sukoharjo, Giyarti said, this time, the cases of bird flu in the region is still in the handling of Mojolaban.  In addition to taking samples of chicken for rapid test at the Great Hall of Verteriner Yogyakarta, relevant agencies have also taken preventive measures.  Department of Agriculture has been vaccinating chickens in Ngambak Kalang and surrounding areas. The Agriculture department has also instructed citizens on how to cull chickens infected with bird flu symptoms in accordance with the burning or burying of chickens.

Since 2003 to 07 May 2013, the number of cases reported from 15 countries worldwide is 629 and 374 were fatal (CFR 59.4%). Among the countries with more than 10 reported cases, Cambodia had the highest CFR of 84.3% (27 out of 32).

Age distribution of the reported cases ranged from 3 months to 81 years old (median 18 years, n=606). The age group with the largest number of cases was 20-29 years of age (22.4%, 136/606), relative to other age groups (0-4; 5-9; 10-19; 30-39; 40-49; 50-59; 60-69; 70+ years). The highest CFR (74.0%) was among persons 10-19 years of age and lowest was among persons 70+ years old (25.0%). 53.8% of the cases were female (325/604). However, age and sex distributions vary by country.
