If you notice that a positive disappeared
from the table, it is because I had marked DS (14) in Sardjito positive from an article before I realized it simply meant a rapid Flu A positive without any reference to H5N1.  I then forgot to change it before posting on Jan 29.  The table on Jan 30 is corrected.

Tamiflu news from Egypt & data from 4,000 patients
{Comment: Are these 4,000 H5N1 patients, or 4,000 regular flu patients, that were treated with Tamiflu?}

Tamiflu never again Effective warded off H5NI

Jakarta, the Stand / Selasa,  30 Januari 2007 / http://tribunkaltim....

Tamiflu was known as the virus charm H5NI. But the worrying news currently emerges. Medical medical treatment with tamiflu on the bird flu patient evidently ineffective. This happened when tamiflu was given to 2 patients who were infected by the virus H5NI in Egypt.

According to the expert in the health Dr Tjandra Yoga Aditama, WHO mentioned that happened because of the virus has bermutasi. Two patients who finally had died were a girl was 16 years old and his uncle who was 26 years old. Both of them died because of being infected by the virus H5NI in the previous month. Realised this, the previous researchers analysed the virus that was received to the two people in the laboratory. And results showed the genetic change in the virus happened so as to become more sensitive and resisten against tamiflu. The similar genetic change happened also to one of the patients in Vietnam.

The data that was gathered from 4 thousand patients who underwent medical treatment with tamiflu, showed in general the level of resistance in the virus happened. To the adult pointed out the figure 0,4 percent and the figure 4 percent to age children 1 till 12 years.

Could you clarify?
When was the Japanese study on 4000 done? Were they all regular flu victims? And why does this article indicate that the results of this testing showed some degree of tamiflu resistance?  Is that resistance in ordinary flu virus or H5N1?  Whole thing is kind of confusing.

Always have a plan B.

I'm trying to find it ;-)
I've come across it, mentioned by Hayden, but I'm not sure it is exactly those figures. Certainly very close.  Incidence of oseltamivir resistance in kids in Japan.  Right now, Lancet is being obstinate and not letting me retrieve that one paper.  I'll let you know whether it is or is not correct when I find it!

More on the fly research from Dr. Wasito
Bird flu was enabled Tertular went through Lalat

30 Januari 2007 / http://www.antara.co...

The solo (Antara News) - the Spread of the bird flu virus (Avian influenza/AI) was enabled through the fly, because of being based on the research was found by the existence of the virus H5N1 in the outside cuticle, the channel pencernakan and in the meat muscle, said Prof Dr Wasito from FK UGM Yogyakarta.

The professor of the Stupid School Of Medicine of the Elephant University (UGM) this proposed that in socialisation gaps had the theme "Bird Flu and AI Anticipation", as well as not frightened of eating chicken and the chicken egg with the Solo community, in Pendapo Big the Surakarta Complex of the Town Hall Office Complex, on Tuesday.

Moreover, according to him, uptil now was proven the fly also became the animal penular for the other illness, like Tuberculosis (the TB), the worm, cholera and desentri.
Former Director General (the Director General) the Livestock Breeding said, in the research that was carried out, evidently the fly could contain the virus H5N1, because this virus was received to the cell of the fly egg that could be unloaded until 25 generations came.He suggested, despite the research was not yet complete and will continue to it was continued, but was not mistakenly in the control bird flu that important was bio-security, that is anti-virus and anti-fly. The AI control that only destroyed the poultry, he said, was really ineffective, except to the poultry that was sick, because if all was destroyed will have an impact that was big also. For the poultry that was sick, he stressed, continued to have to be destroyed and don't disembelih, because poultry blood that was cut off was still containing the virus H5N1. "If having the affected poultry this virus must be destroyed, and don't by means of being cut off, but being given the drugs CO2 then was put in inseminator or the annihilator's implement" of the "poultry."If being burnt disembarang the place as if Indonesia the primitive country. This was for the child's education not good, said Wasito.()

South Korea helps fight bird flu in Indonesia
South Korea helped Indonesia with equipment valued at 500,000 U.S. dollars to fight bird flu in the country, Agriculture Minister Anton Apriantono said Tuesday.

Minister Apriantono said that the devices comprise powder antiseptic, manual knapsack sprayer and devices to rapidly detect bird flu on animals.

The equipment would be used by a number of animal quarantine centers in the country, he said.

Most of the provinces in Indonesia has been infected by the highly pathogenic H5N1.

Sixty-three out of 81 people infected by bird flu have died in Indonesia, all of them were infected through fowls, health ministry said.

Indonesia has slaughtered millions of chickens to prevent the disease from spreading.

I am not convinced that all of them were infected by fowls and I question how he knows for sure.

"The funny incident".....Dr Sardjito - TP/TA (55)
Excerpt:

The chairman of the Team of the Control of Bird Flu of RSUP Dr Sardjito, Dr Sumardi SpPD explained, five people entered time almost simultaneously.
Four residents Gunungkidul entered struck 18.
44 whereas a Yogyakarta resident entered beforehand that is struck 17.
08.

Currently 'we treat 11 patients who were expected by bird flu, was based on the fast test, indeed results' of the 'negative, but we continued to send the sample to Jakarta to know definitely because some time before on his blood was found the poultry died suddenly', he stated.
The possibility in time 48 hours of all the result of the test have been known.

All the patient was expected by bird flu according to Sumardi was treated in isolation space and the capacity him was just right namely 11 beds.
So, if the possibility had the new patient, this room still is not able to accommodate him
.
To 'anticipate him, the patient that results' of the 'test of Balitbang Department of Health the clear Centre' of the 'negative will immediately be moved by us to other space or if his condition has enabled might come home',' he explained.

The funny incident could happen when a patient TP (55 years) suddenly left isolation space, went out towards paths and sat, issued cigarettes and lighted him.
He appeared calm appeared to not care that if evidently he true was affected by the virus H5N1 could be very fatal as a result.
Right how he could slip away from the guarding of the nurse?
It seems according to several nurses, he asked permission to defecate, but evidently turned the direction, sat relaxed and smoked.
During smoked that several reporters asked chatted and he acknowledged not yet old contact with the poultry died.
Several days afterwards he was feverish, the cough and hot high
.

Kulo 'dibeta ting the community health centre, trus direct Ken mriki', he explained that meant him could check to the community health centre but finally was run off with to RS Sardjito.
For a long time did not yet chat with the reporter, the official that since earlier sought immediately asked him to enter again isolation space.
He ngeyel wanted to go out because inside hot and could not smoke.
It was just very strange that the resident Playen, this Gunungkidul, made several visitors keep laughter

Psychological effects
We need to remember that though rare, the Spanish Flu was known to cause psychlological effects in some patients.  John Barry mentions this in his book.

While visiting a nearby town a year or so ago, a person told me a story of a man from that town who killed his wife and chased his childern with a knife while sick with the Spanish Flu back in 1918.  Before getting sick he was a normal, calm individual whom his neighbors would never suspect would behave in such a manner.  His behaviour was attributed to the flu.

The moral of my little (true) story?  Such oddities as is described above are likely personality traits, but...there is a possibility, though remote, of some type of flu influence.

Solicitation for a replacement.
Hi all.  I am going to be unable to track cases in Indonesia for a while, because of other time commitments.  I will finish up through tomorrow, January 31, and then send an updated spreadsheet to Dude to post at the ftp site.  Anyone willing to track is welcome to use the existing Excel spreadsheet or start their own, as desired.

If someone else could take over tracking cases by location beginning in February, then Okieman should still be able to do the maps periodically.  I've enjoyed working with you all and plan to be back, as time allows.

instruction sheet?
Michelle, if you could do a sort of instruction sheet for how you verify what cases to put in the sheet etc., that might help people assess whether they can take on the task and also assure that the quality remains up to your standards during your absence (which I presume has to do with this year's tax season).

I'm considering whether to take it on but am worried that if I do it, there'd be an inordinate amount of argument over, say, whether (hypothetical) SD (10) is or is not the same as SRD (11), simply because of interpersonal history issues.

medical information provided is for discussion purposes only and should not be construed as medical advice. if you believe you have a medical problem, consult your practitioner.

[ Parent ]
In a nutshell, my instruction sheet is to rely on the news searchers
in the majority of instances.  When a case is in question, I try to err on the side of being conservative (must be the accountant in me) so we don't seem to be exaggerating a situation which is already of concern.

I wouldn't recommend someone trying to duplicate the tables or the lists at the top of threads, unless you have an interest in html (which I developed, courtesy of this forum.)  I would, however, recommend collecting as much detail as an article gives about a patient, because it often comes in useful later.

When the tracking started at Old Yeller, we were just all working together to stay straight on names and locations, and it is not much different now, except the volume of cases has increased drastically.  There are lots of formulas in the spreadsheet I used to sort, count, etc... but that was mainly to produce the table and they aren't necessary (just very useful if you enjoy that sort of thing.)

I would recommend someone to start with just basic tracking,  and gradually add other things as you are able.  It takes great patience, on everyone's part, as there will inevitably be some mistakes made.

AnnieB's spreadsheet on the Egypt thread would be a good place to look also, as she is doing a great job with the cases and the tables.

[ Parent ]
Info for maps
The info I use in making the maps is:

1. village, town or city where the patient is from, and the province where it is located

2. test results if available, positive or negative

3. and if the patient has died, I need a note to indicating their death

The spreadsheet Michelle has been maintaining is much more indepth and contains data not shown in the maps.  But for my part, this is all I need to make the maps posted.

[ Parent ]
I am speechless Michelle in OK!
Really sorry you will be tied up for awhile. Wish I could help but I absolutely don't have a clue how you keep all this straight. You do it all so well too. There are a lot of very smart people here and I am sure they will step into your shoes for awhile. We need you so I hope your commitments don't keep you from us for very long.

how do you get the existing excel spreadsheet?

[ Parent ]
ok, I'm NOT going to do it...
I'm thinking over my current situation and realize that I probably shouldn't take on the listing.  1) I attract controversy.  The last thing some people here want to see is *me* deciding what constitutes a new case  2) I'm using a fast computer at the moment but will be going home to a much slower connection in a few days  3) this evening I've developed a runny nose, cough, and now low fever (whee!), so I may or may not be entirely coherent later this week (and if by chance I've crossed paths with patient zero, I may not be *alive* later this week, though I wouldn't encourage anyone to get their hopes up on that.)

So I cede the floor to some other brave soul.

Better today enough to try this, but not yet well. :sigh:

I do think we've (the wiki has) been doing a lot of inadvertent seasonal flu tracking, as commonground observed.  Oh well.

I am currently sick.  Cough, runny nose, variable low fever (100-101), fatigue (as opposed to prostration), and asthma exacerbation.

I don't think I can commit to doing such a thing until at least Sunday, if this takes the usual course of illness for a non-flu URI.  Currently my ability to do the sudoku level that normally takes me 7 minutes has me taking 19 for the same level puzzle; I'm not the one you want making judgements right now.

Anyone else care to give a back-up check for FG until my brain gets out of the shop, meaning today, tomorrow, and Saturday?

Lisa, Sunday is fine
I just need to know that there is another person that can help me follow up with this.

As I told Michelle, Commonground... and I am not sure who else..... many times in the past.  This really is an important project. Others use this data to do different projects.... myself included.  Tons of people have used this lists or graphs to convince their friend and family that this is an issue they should be concerned with.

It does take some amount of preserverence... But, the people it reaches is measured in "lives saved."

Because I am in classes, I have a need for back-up... Not to mention, that most nights I am only getting 5 or 6 hours sleep... I need that other pair of eyes.
So, Thank you.....

One thing that might help Lisa (if she's going to do this)
or whoever does, is if the translators could put a little summary at the top of whatever article they post that gives the kind of information that would go in the spreadsheet: Hospital, township, date of admission, name (or initials) age (at a minimum), plus if this is a new case. If time, they could add other brief relevant commentary. Then Lisa (or?) could just copy and paste that into her spreadsheet.

Always have a plan B.

For whoever volunteers, the news searchers are
already doing this.  It is at this thread which is referenced at the top.

This will mean that the updates might not be as timely as Michelle's but I can give it a go anyway.

Heavy rains coming....(FYI)
COMMENT-theory is that rain makes BF worse.

Jakarta (ANTARA News) - The Meterological and Geophysical Agency (BMG) forecast that heavy rains would shower down on Jakarta and its satellite towns of Bogor, Depok, Tangerang and Bekasi in the next two days.

The rain concentration would take place in the southern parts of Jakarta and its satellite towns, like South Jakarta and the Puncak resort area in Bogor, BMG`s spokesman Ahmad Zakir Zamawi said here Tuesday.

The volume of the raifall in Jakarta and its satellite towns was predicted to reach between 50 mm and 100 mm per day.

The rains in Jakarta were not so heavy in the past week as the volume was only between 30 mm and 40 mm per day but in the next two days, there would be downpours with the volume reaching 50 mm to 100 mm per day.

"In Bogor, the rainfall was heavy on Monday (Jan 29) night as the volume reached 114 mm," he said, adding that in Bogor, rains usually fall in the afternoon or at night while in Jakarta it rains in the morning or afternoon.

He called on Jakartans especially those living near the banks of Ciliwung river to be vigilant against water flows from the river as the peak rainy season would be in January and February, 2007.

He said other areas where heavy rains would fall are the central part and western part of West Java, the northern part and central part of Central Java, and the western part of East Java.

"Heavy rains will also fall in South Kalimantan, South Sulawesi, East Nusa Tenggara and South Sumatra," he said.(*)

Screening
It looks like they are asking for the community hospitals to only reconcile when they have conducted full screening ie including a chest x ray, as well as other symptoms, v=before referring on.

It implies that suspect patients are being immeidiately referred to the main hospital before these investigtions have been completed - increasing their workload.

I would presume that this means that the community hospitals all have xray equipment?

Sensitivity vs. specificity
MIH - on Ol' Yeller there was a thread that detailed sensitivity vs. specificity for testing.  As I can best describe it, if you design a test that is very sensitive, you have more possibility for false positives; if it is very specific, you are going to miss some positives and have more false negatives.  But in that case, the positives you have you really can trust to be positive.  So it's a bit better than a coin toss.
As bad as it is, I do think we can trust the positives; the negatives are another story.  Too many things that can affect specimen integrity.

I'll try and find the link to the old site.

[ Parent ]
Is anyone good at making graphs?
a bar graph comparing suspect cases by hospital area would tell alot, as would a graph showing the suspect cases each day over the month of January.

I'd suggest graphs that removed the "negatives" only we have no idea how accurate they are, so our best bet at this point is to simply leave them in.

I can try to put the data into graph forms, but I'm not sure how I would post it...I am definitely at the C- grade when it comes to computer savvy.

Summarys are good
I've been trying to creat a summary in the discussion section.

A big picture 4 or 5 sentence paragraph is helpful.

Data is good, but needs to be put in context.

I just don't have to time to do any serious analysis with any degree of accuracy.

For example,  there are a lot of "cases",

Very few deaths.

Two new sus cases in Pandeglang Regency / husband & wife

Two was expected by Bird Flu
Ritual [Protest?] Deny the Army in Tangerang

Probably an abandoned hand dug well
Or a well no longer used by anyone.  This was once very common in the US.  Folks would use abandoned wells as places to dump garbage.  It did not occur to them that groundwater moves and what goes in the well contaminates the aquifer.  If it is fairly close and up gradient from a well used for drinking water then it will eventually contaminate the drinking water well.

[ Parent ]
It sounds like only the father passed the chicken well
quote: "Initially my father who was sick.  After passing the place of the disposal of the chicken carcass died, then spread to my mother."

So, the question: where did the mother get it? They apparently got sick one after the other, maybe within a day...could the woman get infected by the man and come down with it that quickly?

Did it mention how many days had passed from the time the father passed by the chicken well to when he got sick?

Always have a plan B.

New case/death - Joni Iskandar (10)

Wayan expected this child terjangkit bird flu. This was seen from the characteristics of casualties who experienced the high and breathless fever.

"When arriving here, his condition has been very critical." I only could handle his 10 previous minutes died, said Wayan. Casualties's father, Amber, said his son since last week the high fever. However he did not carry him to the hospital because of thinking only common fevers.
"Beforehand I was not suspicious." Thought by me my child is sick normally because of the season pancaroba this, he said. But, increasingly the temperature day of the Joni body increasingly high and since the last three days was seen by the breathing difficulty. At "that time was seen by me my child the difficulty breathed, as being affected by asthma," he said. Saw the situation, Amber immediately brought to the Wayan Pasek house to immediately get the maintenance. However, Joni could only remain for 10 minutes and died in the Wayan house.

The assumption of the death of casualties resulting from bird flu also was sent by the Multicoloured Village Head, Amir Trilangka. "Currently, with the incident that struck the Amber family, our resident was very restless and worried," said Suyanto, the local public figure. So, he asked the government immediately to anticipate the possibility of the outbreak of the bird flu virus of not infecting the other resident. I will forward the Jan 31 spreadsheet to Dude tonight.
It may take a day or so for it to get posted.

As you've seen, I started a Feb 1 diary without the case list to give someone a chance to get on their feet tracking before trying also to maintain that list.  If you will start the next diary, when Dem or another Mod has time, they can paste in the codes for the maps, etc.  Then all you have to do in starting a new diary is to copy it over and change the "prior diary" link.

The last sheet in the Excel spreadsheet contains the html code for the case list.  I just copy/paste from the 2007 tracking list to that sheet by hospital.  Then I copy/paste from the spreadsheet into the diary.  That part, however, is very time consuming.  I've used color codes daily as I'm tracking to trace which changes were posted there and which weren't.  You may or may not want to continue that.

The second sheet in the spreadsheet has the code for the nightly table.  The Excel formulas are a little difficult when adding a new month, so I'll try to get Feb set up if you want to use them.  There are really too many changes to track manually.

Best wishes to you.  If you have questions, just respond to this comment and I'll try to check back to answer for the next few days.

Michelle, Thank you ... I hope
I can do half the job that you did... because I only know about half as much about excel as you...

I will keep the list going, and I will try to keep links on a separate copy so that if there is a question, then we can find it easier.

I am near the end of one class, and my next class will either begin at the end of February or the end of March.  That will probably not be as ideal as we would hope, since tax season is not yet over.

Yes, please stick around for questions and everything else.  I am sure I wll have many!.

I will see if Dude has posted the spreadsheet yet...

Michelle, I got the spreadsheet from Muse
Thank you... So looking at it I have a couple of questions to start with.

1.  It appears you are now keeping the spreadsheet with an upper portion of patients that have either been released or have died....  Is that right?

2. Summary Sort # Month Table
What are these headings?  Are these just for sorting?
Would you mind explaining it so I do not guess wrong...  I have an idea.... but....

3.  Under Vent/ICU  you have just been putting a "*" to indicate that they were in ICU at some time... Is that correct?

Ok.. enough for now..... My last 2500 word paper is due tomorrow night... (No, I haven't started it  :) )...
I just have a project due next week, that I will be working on. By Sunday, I will be able to devote more time to this... and Lisa did say she would help.

OK... Let me see if I can get hold of the links to stuff... I will need to figure out how to build tables on Sunday.  Thank goodness I already figured out how to post that graph... (with Dem's help of course.).

[ Parent ]
FloridaGirl, you're doing great.
To answer your questions, as best I can:

1.  I did split out the upper portion for patients who had been released or died, but that was really only for my use in creating the list at the top of each thread.  I had fewer lines to sort through that way.  The red descriptions I only added for your use.  If I had continued with the spreadsheet, I would have deleted the red lines and then resorted from time to time by the column called "released" to get the separation again.  I didn't move people up manually.  I just added blank lines between the two groups for visual effect.

Note:  in sorting, I always pointed at the row number to the far left and highlighted down to ensure all fields were captured.  If you are sorting all the data, you can include the heading row (and mark the heading box) to make the labels appear.

2.  I started that method way back in 2006 when I was trying to become familiar geographically with Indonesia.  I printed a map from Wikipedia of the provinces and used Encarta to try and create a list on the "Headings" tab that would go from upper Northeast, down through Sumatra and Java islands, and back up through Sulawesi.  When new locations showed up, I would insert rows on the headings sheet in the correct geographical locations, and then highlight the top numbers in column D on that sheet and drag down to renumber everything.  In this manner, when the formulas in the pink column on the main sheet were maintained, I could sort by that Sort # and all locations in close geographic range would be grouped together. Otherwise, if I tried to sort by the typed in location, they were alphabetical and had no useful order.

Note:  Here lately, there were so many new locations that I didn't have time to put them in the correct geographic order within their region.  I was doing good to just get W. Java, etc. grouped together.  This list can be rearranged at any time and the effects will flow through to the main page.

3.  Yes, that is correct and started being done per Susan C's request.  I only started mid-January, so patients in early January weren't done.

One last note, the lines in bold blue are for Okieman's use.  They are the lines which have been added since he last did a map.  (We couldn't really use dates, because sometimes patients are reported retroactively and we would miss them.)  If you keep them in blue and make new patients you add in blue, then when you have Dude load the spreadsheet, Okieman can sort by location and easily tell which patients are new.  Once you send in the spreadsheet, make all the lines normal font and start the process over again.  (This also helps you  quickly identify recent cases to prevent duplicates from occuring.)

Thanks again for taking on this work.  I hope your paper and final projects go well.  I'll keep checking back here to answer questions.

[ Parent ]
Michelle, AnnieB, or Mama,
A couple of questions if you do not mind.

1st.  When you are sorting for the table.  The columns for Jan. Feb ... and so on... What is the criteria for the patient being placed in which month?

Is itdate of admission or date of the test results back?

In other words...  Patient is admitted in January (also onset of symptoms), is tested,..... first test results are negative..... gets retested IN FEB ...... the second test results are postive....  Which column would that patient be in? Jan or Feb?

Would he have already been counted in Jan? and that needs to be changed?  Change the n to p or +d in the sort column?

Boy, I know Michelle's mind is filled with these details!

Changing her methodology would mess up those numbers and I do not want to do that.

FloridaGirl, I have sent you an email
to your "singtome" address - not sure how often you check it.

I need to understand which "table" you are updating - the one that also goes into the News Diary or the big "all cases, ongoing" one?

I cn send you the coding for the "News Summary" table to save you re-inventing it.

As to your question - I do not know what Michelle's "rules" were - I would be guided by WHO for positive cases - they record for the month that the result is given, so, in your example, they would come off the Jan -ve tally and go on the Feb =ve tally - hope that makes sense and helps ;-)

FloridaGirl, about the table...
I simply used the first date shown for a case (whether it was symptoms, hospitalized, or reported) to assign them a month in the table.  Once in January, they stayed in January, even if they got test results back in February.  My reasoning for this was to keep any potential outbreaks grouped together by the time of the illness, regardless of later developments.  That is why the January numbers could change, even in February.

Also, to AnnieB or anyone else able to access and edit the February 1 diary.  If you will copy over the entire top section (not the extended part, just the top) to the latest Indonesia thread, it will add the table with Okieman's latest maps (dated Jan 26th, I think).  Then FloridaGirl can copy it to her next diary.  The only thing that needs to be changed is the reference to the prior thread which will come after the "blockquote" codes.

FloridaGirl, I hope your final class projects are going well.  Thanks again for taking on this additional work.

Thx Michelle in OK
I have already copied the pics etc for FloridaGirl - it is just the Table now and I wanted her to be "ready" first ;-)

Cheers and thanks and I hope is all going well for you.

AnnieB, about the pics...
Only the pics that were in the 2nd section of the diary were copied over.  They were the earlier pics which were posted only to give a picture of the change over time.

The latest pictures in the top section were not brought over.  They should also show up on the front page (if it is done like the Feb 1 diary.)

Thanks for all of the effort.  I really appreciate you all stepping up to do this.
